Job summary
Eltham Medical Practice is looking for an experienced and motivated Advanced Nurse Practitioner (ANP) to join our friendly, forward-thinking primary care team. This is an exciting opportunity to work within a large, well-established practice that values innovation, teamwork, and high-quality patient care.
The successful candidate will play a key role in delivering autonomous clinical care, supporting our multidisciplinary team, and contributing to ongoing service development as we continue to expand our patient list.
Main duties of the job
Provide high-quality clinical assessment, diagnosis, treatment, and management of patients presenting with a wide range of undifferentiated and undiagnosed conditions.
Undertake acute illness / disease assessment and management, including prescribing and reviewing medications within scope of practice.
Support urgent on-the-day demand alongside the duty team.
Deliver proactive, patient-centred care for long-term conditions in collaboration with the wider clinical team.
Request and interpret investigations, make referrals, and liaise with other health and social care professionals as appropriate.
Contribute to clinical governance, quality improvement, and the development of care pathways.

Salary:£60,000-66,000 base range dependent on experience (negotiable). Hourly Locum rates available £40 per hour

- Proven ability to assess and manage acutely unwell and chronically ill patients in a primary care setting
- Experience in practices using triage, prescribing, and working within a multidisciplinary team

- Experience supervising students or junior clinicians
- Experience with EMIS Web clinical system
- Interest or expertise in specific long-term condition management (e.g. asthma, frailty)
