Job responsibilities

Key Responsibilities:

1. Patient Observations:

- Conduct regular observations of patients, including monitoring vital signs, temperature, pulse, and respiration rates.

- Report any changes in patients' health status to the healthcare team and record on patients notes.

2. Blood Sample Collection:

- Perform venipuncture and capillary blood sampling as required.

- Ensure samples are collected, labelled correctly, and transported to the laboratory in compliance with protocols.

3. Patient Care:

- Make assessments of a patient's home surroundings, to offer social and mobility support.

- Discuss Future Care plans, support comprehensive geriatric plans, and personalised care plans.

4. Administrative Tasks:

- Maintain accurate records of patient observations and care delivered.

- Ensure compliance with health and safety guidelines and organisational policies.

5. Collaboration with Team Members:

- Work closely with the Non-Clinical Home Visiting Service Lead and other healthcare professionals within the service and Primary Care Network to coordinate patient care.

- Participate in team meetings and training as required.

You will be required to work from both our Hayling Island site and Emsworth site.

Person Specification

Qualifications

Essential

• Trained in phlebotomy.

Desirable

• Relevant certification or training in health care support (e.g., NVQ Level 2/3 in Health and Social Care).

Experience

Essential

• Previous experience as a Health Care Assistant or in a similar role is preferred.
• Strong interpersonal skills with a compassionate and patient-centered approach.
• Excellent communication and organisational skills.
• Ability to work independently and as part of a team.
• Valid driver's license and access to a vehicle for home visits.
