Job responsibilities
Clinical
To work as part
of the Primary Health Care Team across all member practices within the Hayling
and Emsworth Primary Care Network. The Clinical Pharmacist role includes and is
not limited to:
-Clinical medication reviews including post hospital
discharge reviews
-Polypharmacy reviews
-Answering medicine
queries
-Medicines reconciliation
-Supporting incentive schemes
-Supporting public health campaigns
-Medicines optimisation including repeat dispensing
-Contributing to QOF and PCN service specifications
-Clinical audit
-Long Term Condition
clinics
-Streamlining repeat prescribing process
-Participate in multidisciplinary meetings regarding
specific patients e.g. Intermediate Care Team
meetings
-Participate and deliver education and training to
staff within the PCN regarding medication related topics
-Facilitate good working relationships between the
PCN and Community Pharmacies and Secondary Care Pharmacists
-Ensure that the PCN performs well
against local Prescribing Indicators and Prescribing Budgets
-Take responsibility for remaining clinically
up to date and attend training events as appropriate to ensure continued development
-Enrolment on the
Primary Care Pharmacy Education Pathway
Training and personal development
-To maintain registration as a Clinical Pharmacist
and comply with appropriate professional codes.
-Training requirements will be monitored by yearly
appraisal and will be in accordance with PCN requirements. Personal development
will be encouraged and supported by the PCN. It is the individuals
responsibility to remain up to date with recent developments.
-There will be ongoing professional
mentoring from a GP lead and with duty GP rotas on a daily basis.
-Participate in the education and
training of students of all disciplines and the introduction of all members of
PCN staff where appropriate.
-Maintain continued education by
attendance at courses and study days as deemed useful or necessary for
professional development.
-Attend the regular clinical meetings
with involvement in clinical education, patient case awareness and prescribing updates.
-If it is necessary to expand the
role to include additional responsibilities, full training will be given.
-Develop and maintain a Personal Learning Plan and
incorporate revalidation tasks.
Communication
-Demonstrate sensitive communication styles to ensure
patients are fully informed and consent to treatment.
-Communicate effectively with patients and carers,
recognising the need for alternative methods of communication to overcome
different levels of understanding, cultural background and preferred ways of communicating.
-To discuss continued development of the service to
promote best practice and a high standard of care with GP Partners, Nurse Leads
and general management teams.
-To communicate effectively with Nurse leads, working
to build good relationships within nursing teams as a whole. Attending nurse
meetings and training as appropriate.
-To communicate effectively with admin teams to
ensure good relationships and the needs of patients are met.
Managing Risk
-Manage and assess risk within the areas of
responsibility, ensuring adequate measures are in place to protect staff and patients.
-Use developed communication, negotiation and
conflict management skills to overcome barriers.
-Monitor work areas and practices to ensure they are
safe and free from hazards, and conform to health, safety and security
legislation, policies, procedures and guidelines.
-Take all reasonable personal precautions necessary
to avoid any potential health risks to colleagues, people receiving care and
the public.
-Apply infection control measures within each
practice according to local and national guidelines and in accordance with the
practice policy.
-Be aware of current evidence, knowledge and
developments in reducing mistakes and the effect of them, and the impact of
human factors and system failures.
Liaison
-There is a
need to work closely with non-clinical teams including reception and
administration staff to ensure the smooth running of each Practice. Reporting
of any problems encountered should be made to the relevant manager.
-There is
also the need to establish and maintain good liaison with other surgeries and
agencies including secondary care and our wider stakeholders.
Other Duties
-To help cover
staff holidays and sickness when required.
-To participate in regular staff training sessions to
ensure development of skills and knowledge; to assess own learning needs and
undertake learning as appropriate.
-To participate in nursing, clinical,
multi-disciplinary and whole team meetings as required.
-To observe and comply with all appropriate statutory
requirements in relation to Child and Adult Safeguarding and to be familiar
with the reporting process should this be required.
-To ensure that policies and procedures published on
the IG Compliance Toolkit are read regularly.
-To contribute to policy development and
implementation of procedures and guidelines in relation to their own area of work.
-To participate in an annual performance review and
to manage individual revalidation process.
-To carry out any
other reasonable duties which may be requested from time to time.
-This job description is not exhaustive and is

patients entrust us with, or allow us to gather, sensitive information in
relation to their health and other matters. They do so in confidence and
have the right to expect that staff will respect their privacy and act
appropriately.
-In the performance of the duties outlined in this
Job Description, the post-holder may have access to confidential information
relating to patients and their carers, practice staff and other healthcare
workers. They may also have access to information relating to the PCN as a
business organisation. All such information from any source is to be regarded
as strictly confidential.

colleagues, other healthcare workers or the business of the PCN or individual
practices may only be divulged to authorised persons in accordance with PCN and
individual practice policies and procedures relating to confidentiality and the
protection of personal and sensitive data.

- George and the Dragon Surgery, The Square, Westbourne, Emsworth PO10 8UE.
- Emsworth Medical Practice, 6 North St, Emsworth PO10 7DD.
- Hayling Island Health Centre, Elm Grove, Hayling Island PO11 9AP.
