An exciting opportunity has arisen to join our
innovative and dynamic Frailty Team in the community. As an experienced and
motivated Band 8AAdvanced Clinical
Practitioneryou will be passionate about service development and providing
a high standard of patient centred care.
You
will have a key role in supporting the work of PCN in transforming the care of
older, frail and more dependant patients, and will support the practices meet
the requirements of the care home DES. You will proactively case manage patients requiring
frailty advice, addressing their unmet needs, and improving their care
coordination and wellbeing.
As a specialist nurse, you will visit patients in
their home / care home, using a person-centred approach to undertake a
comprehensive needs assessment. Your clinical decision making will be made
through assessment and interpretation of clinical information to support frail
and elderly patients having an acute exacerbation of a frailty related
condition.
This
role is 37.5 hours to be worked across 5 days.
A
full clean driving licence and use of your own vehicle is essential for this
role.
Main duties of the job
The post holder will be an autonomous practitioner who will take an
active role in responding, stabilising and make safe element of acute care
pathways by delivery of advanced assessment, and the management and treatment
of patients over 65 years of age referred to the PCN Frailty, both living in
the community and in care/nursing homes.
The
Advanced Clinical
Practitioner will assist patients to proactively
manage and anticipate their future health needs and develop appropriate plans,
in conjunction with their GP and other medical staff as well as Allied Health
Professionals, Nursing and Social Care staff to maximise their independence and
develop health plans to manage future health problems and support self-care.
The
post holder will lead in developing, delivering, and evaluating high quality
patient centred interventions in conjunction with the associated clinical teams
to ensure this is incorporated within both the overarching PCN Frailty Service
and the St Helens Local Care Organisations objectives.
The Advanced
Clinical Practitioner will have a key role in supporting the work of PCN in transforming the
care of older, frail and more dependant patients, delivering integrated out of
hospital care for patients. They will support the practices meet the
requirements of the care home DES and in the delivery of the NHS Long term
Plan.

as part of a multidisciplinary team and across the primary, secondary, and
social care interfaces in St Helens, the post holder will be responsible for
co-ordinating the delivery of urgent health care needs of people at risk of
hospitalisation or requiring a facilitated timely discharge and providing
proactive advanced assessment of the practice population. This includes direct
referrals from GPs, Care homes, and proactive post discharge reviews.
The post holder will provide
highly specialised health assessments and input as an independent and
autonomous practitioner developing and leading relationships with members of
the multi-professional team allowing for consultation and collaboration in a
variety of health care settings.
The focus will be to provide
ongoing medical support and prevent unnecessary acute hospital admissions and
enable patients to remain safely and supported within their own homes/nursing
and residential homes.
To assist patients to proactively
manage and anticipate their future health needs and develop appropriate plans,
in conjunction with their GP and other medical staff as well as Allied Health
Professionals, Nursing and Social Care staff to maximise their independence and
develop health plans to manage future health problems and support self-care.
