Job summary
St Helens Central PCN are looking to recruit two qualified Nursing Associates (part time and full time positions) to work within our Primary Care Network multidisciplinary healthcare team supporting the Frailty Team.
This is an exciting role where you will be using your skills to support and complement our existing team providing and monitoring patient centred care, across the patient population of St Helens Central network.
The post holder will be based within the frailty team covering the Practices within the network. You will be required to travel from base to care homes/patient homes and to other venues during fulfilment of their duties, therefore a full clean driving license and access to a reliable vehicle insured for business use is essential.
You will be a Registered Nursing Associate, who is motivated, proactive, and committed with excellent communication skills. You will work independently and within the Nursing Associate team under the leadership of our ANP, supporting with all aspects of nursing care and improving on the safety and quality of patient care at every opportunity.
The role will involve working within core hours 8am - 6:30pm however a degree of flexibility is required, there may be a future requirement for the team to provide more flexible appointments, including extended access hours and Saturdays.
We welcome applications from qualified Nursing Associates who hold a UK PIN number, have access to a car with a full clean driving licence.
Main duties of the job
The post holder will assist and perform routine tasks related to patient care, such as blood pressure checks, BMI lifestyle and advice, as directed and agreed with line manager / GP.
They will provide care and support to patients in practices, patients own home, care homes or other settings as part of planned outreach programmes. Support remote assessment of patients by other clinical staff using digital technology.
To support patients to improve and maintain their mental, physical, behavioural health and well-being by using behaviour change interventions, such as undertaking health checks (physical and mental, smoking cessation services.
To undertake administration of prescribed medicines via IM SC routes, following competency based training and practice policy.
Provide care for individuals with dementia, mental health conditions, and learning disabilities to support health action planning, as part of Learning Disability and Severe Mental Illness, physical health checks, working closely with the Network LD and SMI care coordinators, with support of practice clinical leads and community specialist teams.
To support chronic disease management including but not limited to monitoring patients with stable asthma, COPD and undertaking preliminary investigations for Type 2 diabetes patients and heart disease, such as urine analysis and arranging ECGs.
Previous primary care experience is preferred but not essential as training and support will be given.
