Job summary

Thorpe-Le-Soken Surgery are seeking an Advanced Nurse Practitioner to join our friendly, supportive and caring team. The role will offer you the opportunity to build on your experience of working effectively within a multi-disciplinary; healthcare team, as you will work alongside fellow professionals within the practice in support of one another.

Main duties of the job

Duties and Responsibilities

Clinical

Receive patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs creating individual care plans.

Assessment, diagnosis, treatment, prescribing for acute minor illness appropriately and deliver key elements of NSFs, providing public health, health protection and promotion programmes that improve health and reduce inequalities.

Dealing with sensitive matters such as End of Life and speaking to patients/parents/carers and provide appropriate ongoing support to meet identified needs and refer on as necessary. Attending End of Life meetings within the surgery.

To ensure significant events are reported using Practice procedures

To maintain line manager responsibility for the nursing team, including staff development, training and maintaining staff cover.

We use EMIS Web clinical system along with Ardens templates and systems. We consult with our patients using a range of methods from face to face, video, telephone. We also have a branch surgery in Kirby Cross and all our staff work across both sites. Our practice offers the full range of primary care services and our highly experienced nursing team have implemented a system for chronic disease management.

To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice.

13.

To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice.

Attend and participate in staff meetings as required.
