We are looking for a Mental Health Practitioner, to join our PCN as a Band 6 equivalent (dependant on experience). You will be a qualified Nurse or Allied Health Professional, with the relevant professional registration (NMC or HCPC). You will be employed directly by the PCN.

You will have a proactive approach to innovation and be passionate about providing a high-quality holistic approach to the care. You will be well supported within the PCN, within the emerging West Independent Care Area (ICA), which has a strong collaborative ethos, and across Cornwall as appropriate.

Main duties of the job

You will be based and embedded within our network practices working as an integral part of our clinical teams, alongside other professionals including fellow mental health practitioners and social prescribers, as part of the wider primary care Multidisciplinary Team (MDT). You will work with primary care GPs to manage the mental health needs of their population at the point of contact. This includes those with lower-level support requirements that can be met within primary care.

Our mental health team manages a significant proportion of patients presenting with mental health issues to the PCN practice surgeries. We are working towards a self-triage system for the mental health and social prescribing team. You will manage patients presenting with a range of clinical priorities, formulate diagnoses and new treatment plans or review and adapt pre-existing ones. This is a collaborative role with practice clinicians, secondary care, talking therapies and third sector organisations.

About us

The PCN patient population is circa 40k spreading across a large area of mid-west Cornwall from our glorious coastline to our heritage moorland and mines. There are two member practices: Carn to Coast Health Centres and Godolphin Health. Our PCN is innovative and embraces 'Modern General Practice', being one of the early adopters in SW England of the Artificial Intelligence powered triage system KLINIK. A system designed to support clinician triage, patient navigation and improve both the patient and staff experience. Our mental health team support the practice duty teams in managing on-the-day demand as well as supporting those patients who can be effectively and appropriately managed within primary care with clinically required follow-up.

JOB SUMMARY:

Provide clinical expertise and support to GPs and practice clinicians to manage patients with mental health problems in primary care. From urgent presentations to longer term management, including the review of pre-existing mental health plans and medication to the formulation of diagnosis, treatment planning and prescribing. You will work in collaboration with a range of practice clinicians, fellow mental health practitioners, social prescribers, community health and wellbeing workers, secondary care, talking therapies and third sector organisations.

The post holder will always work to promote the safety and the wellbeing of these patients and their families/carers.

Support to undertake non-medical prescribing is available. The role may evolve to include education and training of clinical and non-clinical staff and trainees.

MAIN DUTIES AND RESPONSIBILITIES:

The duties and responsibilities of the post holder are detailed below, although the precise accountabilities of any job may be expected to change over time. Job holders would be contacted over any proposed changes before implementation.

  • Triage and assessment of patients presenting with mental health problems
  • Proactive follow-up of adult patients with serious mental illness both in terms of mental and physical health
  • Annual medication reviews for those on longer term antidepressants and antipsychotics
  • Non-medical prescriber or opportunities to undertake prescribing course or other education programmes
  • Active signposting to third sector organisations/ engage with in-house social prescribing team
  • Attend fortnightly mental health meetings with local CMHT and IAPT.
  • Help where appropriate to co-ordinate support for the more vulnerable groups of patients experiencing mental health problems e.g. the homeless, substance misuse, learning difficulties
  • Consultations to be undertaken with patients both in surgery, over the telephone, video or home visits.
  • Capable of working independently and proactively
  • Active management of flexible caseload with awareness of dependency
  • Protection of Children and Vulnerable Adults. To promote and safeguard the welfare of children, young people and vulnerable adults.
