Are
you a qualified Prescribing Clinical Pharmacist looking to work within an
innovative forward-thinking primary care team, prepared to lead fellow
pharmacist(s) and a great team of pharmacy technicians?
We
would welcome applications from existing/aspiring Senior Clinical Pharmacists.
You would be working in a diverse and supportive clinical team with an
organisational culture that encourages training and development. Ideally, your
experience will have been gained within primary care or another healthcare
setting. The successful applicants will be joining an established team with a
fellow pharmacist (currently on mat leave)and a great team of pharmacy technicians, in a very
harmonious PCN comprising only two member practices.
The
successful candidate/s will have access to an accredited training pathway that
supports the pharmacist to practice and prescribe safely and effectively in a
primary care setting and to deliver the key responsibilities of the role. As
part of this training pathway the candidate will be introduced into a
supportive network of pharmacists across the region.

Main duties of the job
You will work as an integral part of the clinical teams at the practices, leading and mentoring your fellow pharmacy colleagues, with a strong relationship with the Clinical Director/practice lead.
You will lead and engage with our
clinical teams to effect planned changes around workload management and prescribing safety.
As part of a valued member of the
skillmixed clinical team, you will support the practices clinicians by
managing clinical workload within your and your fellow team members scope
of practice/expertise and capacity.
Your day will be varied to include:
- advising patients on the use of medicines
- medicines optimisation
- improving patient safety through quality
initiatives and audits
- structured medication reviews
- supporting care homes
- supporting and developing pharmacy team members
About us
The PCN
patient population is circa 39k spreading across a large area of mid-west
Cornwall from our glorious coastline to our heritage moorland and mines. There
are two member practices: Carn to Coast Health Centres and Godolphin Health (Praze-an-Beeble/Connor
Downs Surgeries). Our two practices work very collaboratively and harmoniously with a strong supportive team ethos across the PCN. A forerunner
of Modern General Practice, we have utilised technology to optimise the patient
pathway. For example, the PCN practices
were early adopters (2020/21) of Artificial Intelligence powered triage system
KLINIK designed to support clinician triage, patient navigation and improve
both the patient and staff experience some of these calls will be
appropriately handled by our pharmacy team.
The
post holder will be well supported within the PCN and within the West
Independent Care Area (ICA) which has a strong collaborative ethos and an
established pharmacist forum across several PCNs. Our PCN is very proud of our Pharmacist Team
and will continue to explore the possibilities together for the growing the
roles within primary care.
We
