Job summary

With the advanced development of Primary Care Health Networks (PCNs) in Tower Hamlets, PCN 6 Bromley by Bow & Stepney Health (B&SH) CIC and PCN 5 Bow Health Network CIC are looking for a highly motivated, innovative person to join our General Practice Teams as a Same Day Access Administrator we are looking for this role to start immediately.

Main duties of the job

The role is pivotal in ensuring all patients receive the best possible care and service. The Same Day Access Administrator role will support the Practices within both PCN in coordinating all key activities including access to services advice and information, and ensuring health and care planning is timely efficient and patient centred. The role will include supporting in patient triaging and booking patients into practice slots carrying out tasks from the GP and supporting the monitoring of the service.

Job responsibilities

Overall purpose of the post

To work closely with all the practices within PCN 5 and 6 and the same day access team to provide a central coordinated role in triaging and booking patients into practice slots carrying out tasks from the GP and support the monitoring of the services.

Key responsibilities of the post

To provide a single point of access for staff and service users

To actively manage patients care plan delivery and liaise with the PCNs clinicians and Allied Professionals as needed

To help people to manage patients needs through answering queries making and managing appointments

Manage and deal with incoming healthcare referrals promptly adhering to company timescales

To help support and coordinate monthly quarterly annual dashboard data KPIs and additional target related performance reports

Interpret data and communicate data outcomes

To maintain accurate patients records

Attend all relevant and required training and comply with Network arrangements for continuing professional development

We aim is to keep quality high services equitable and bureaucracy low in a partnership of mutual trust respect and vision

You will be part of a team dedicated to the delivery of high quality health care whilst instigating engaging in initiatives to tackle the wider determinants of ill health

  • Experience in a reception/administration role
  • Evidence of consistent pattern of learning from education, training and experience

Experience

  • Experience in Primary Care or social care
  • Ability to communicate confidently both with clinical and non-clinical staff
  • Good time management and prioritisation skills
  • Ability to work on own initiative
  • Adaptability and a flexible positive approach
  • Willingness to undertake further learning and training
  • Experience of developing relationships with a wide variety of people
  • Demonstrate commitment to team working
  • Good IT skills
  • Fitness to do the job within manual-handling policy
  • Ability to work with high volumes of patient contacts and a pressurised environment on a daily basis

  • Experience of working with Health IT systems such as EMIS & AccuRx
  • Experience and knowledge of working with communities within Tower Hamlets
