Clinical Pharmacist Full Time (37.5 hours) Winchester Rural South PCN

Winchester Rural South PCN is seeking a motivated and clinically focused Clinical Pharmacist to join our multidisciplinary primary care team. This is a full-time post (37.5 hours per week) and is available on either an employed or self-employed basis, offering flexibility for the right candidate.

Salary isdependent on experience.

This role is delivered in line with the NHS 2025/26 Network Contract DES and focuses on providing high-quality clinical pharmacy services across the PCN.

Key responsibilities include:

  • Delivering Structured Medication Reviews (SMRs) for patients with complex polypharmacy, frailty, long-term conditions, and high-risk medicines

  • Providing expert medicines advice to GPs, nurses, and the wider PCN multidisciplinary team

  • Supporting medicines optimisation, including deprescribing and cost-effective prescribing

  • Managing medication-related queries, medicines reconciliation, and repeat prescribing processes

  • Supporting long-term condition management, including cardiovascular disease, hypertension, diabetes, and respiratory conditions

  • Identifying and managing patients at risk of medicines-related harm

  • Supporting care home medication reviews where required

  • Contributing to clinical audit, QOF, IIF, and PCN priorities

  • Working collaboratively across multiple GP practices within the PCN

  • Supporting prescribing governance, medicines safety, and quality improvement initiatives

Job Purpose

The Clinical Pharmacist will work as part of the multidisciplinary team within Winchester Rural South PCN to deliver high-quality, patient-facing and non-patient-facing clinical pharmacy services in line with the NHS 2025/26 Network Contract DES.

The post holder will support medicines optimisation, improve patient outcomes, reduce medicines-related harm, and support safe, effective, and cost-efficient prescribing across the PCN.

Key Responsibilities
  • Deliver Structured Medication Reviews (SMRs) for patients with complex polypharmacy, frailty, long-term conditions, and high-risk medicines

  • Provide clinical medication advice and support to GPs, nurses, and the wider multidisciplinary team

  • Support medicines optimisation, including deprescribing, switching, and improving cost-effective prescribing

  • Manage and resolve medication-related queries, including medicines reconciliation following hospital discharge

  • Support repeat prescribing processes in line with local and national guidance

  • Identify patients at risk of medicines-related harm and implement appropriate interventions

  • Support long-term condition management, including cardiovascular disease, hypertension, diabetes, and respiratory conditions

  • Undertake care home medication reviews where required

  • Contribute to clinical audit, quality improvement projects, QOF, IIF, and PCN priorities

  • Work collaboratively across multiple GP practices within the PCN

  • Support prescribing governance, medicines safety alerts, and policy implementation

  • Maintain accurate and timely clinical records using GP clinical systems

  • Participate in ongoing training, appraisal, and professional development
