The Clinical Pharmacist will work as part of the multidisciplinary team within Winchester Rural South PCN to deliver high-quality, patient-facing and non-patient-facing clinical pharmacy services in line with the NHS 2025/26 Network Contract DES.

The post holder will support medicines optimisation, improve patient outcomes, reduce medicines-related harm, and support safe, effective, and cost-efficient prescribing across the PCN.

• Deliver Structured Medication Reviews (SMRs) for patients with complex polypharmacy, frailty, long-term conditions, and high-risk medicines

• Provide clinical medication advice and support to GPs, nurses, and the wider multidisciplinary team

• Support medicines optimisation, including deprescribing, switching, and improving cost-effective prescribing

• Manage and resolve medication-related queries, including medicines reconciliation following hospital discharge

• Support repeat prescribing processes in line with local and national guidance

• Identify patients at risk of medicines-related harm and implement appropriate interventions

• Support long-term condition management, including cardiovascular disease, hypertension, diabetes, and respiratory conditions

• Undertake care home medication reviews where required

• Contribute to clinical audit, quality improvement projects, QOF, IIF, and PCN priorities

• Work collaboratively across multiple GP practices within the PCN

• Support prescribing governance, medicines safety alerts, and policy implementation

• Maintain accurate and timely clinical records using GP clinical systems

• Participate in ongoing training, appraisal, and professional development

• Maintain GPhC registration and work within professional standards and scope of practice

• Work in accordance with PCN policies, NHS guidance, and information governance requirements

• Contribute positively to a supportive, learning-focused PCN environment

The post holder is accountable to the PCN Clinical Director / designated GP supervisor and will work closely with practice teams and other ARRS roles.

Person Specification

Qualifications

Essential

• Registered pharmacist with the GPhC
• MPharm degree or equivalent pharmacy qualification recognised in the UK
• Evidence of continued professional development (CPD) relevant to clinical pharmacy practice

Desirable

• Independent Prescriber (IP) qualification or working towards / willingness to undertake training
• Masters degree in Clinical Pharmacy, Primary Care Pharmacy, Advanced Clinical Practice, or a related subject

Experience

Essential

• Post-registration experience as a registered pharmacist
• Experience providing clinical pharmacy services within primary care, general practice, or community pharmacy
• Experience of medicines optimisation and managing patients with multiple long-term conditions
• Experience of handling medication-related queries and providing medicines advice to patients and healthcare professionals
• Ability to work independently and manage a clinical workload while contributing effectively to a multidisciplinary team
• Experience of using clinical systems and maintaining accurate clinical records

Desirable

• Experience working as a clinical pharmacist in primary care or general practice
• Experience delivering Structured Medication Reviews (SMRs)
• Experience working across multiple GP practices or within a PCN
• Experience supporting long-term condition management (e.g. cardiovascular disease, hypertension, diabetes, respiratory disease)
• Experience of care home medication reviews
