A vacancy has arisen for a Practice Nurse at Saxonbury House
Medical Group.
We are a busy GP surgery and vaccination centre located in
Crowborough, East Sussex. We are seeking to recruit a nurse who will work as
part of a team delivering nursing care and clinical support for patients living
with long term conditions as well as immunisations and cervical cytology.
The post is part time up to 25 hours per week and we can be

as required.

our friendly and supportive GMS practice covering 9,200 patients in
Crowborough. Crowborough is situated on the edge of Ashdown Forest, in the High
Weald Area of Outstanding Natural Beauty 7 miles south-west of Royal Tunbridge
Wells and 35 miles south of London, with excellent road and rail links.

Job Summary
The post holder is responsible for the delivery of basic practice nursing services and care to the practice population. Supported by senior nurses within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health promotion / prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
Key responsibilities
Clinical practice
- Assess, plan, develop, implement and evaluate treatment or programmes to promote health and well being.
- Implement and evaluate individual treatment plans for patients with known long-term conditions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of emergency care
- Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self care
- Deliver opportunistic health promotion using opportunities such as new patient medicals
- Assess and care for patients presenting with uncomplicated wounds
- Support and manage health needs of women presenting for cervical cytology consultations
- Implement and participate in vaccination and immunization programmes for both adults and children
- Advise, support and where appropriate, administer vaccinations for patients travelling abroad
- Promote and deliver care for patients presenting with aural conditions
- Assist doctors in providing minor surgery sessions
