Job summary
To work as part of the Workington Wound and Vascular service providing patient centered, nurse-led holistic assessment and wound management, where appropriate, to the registered population of Workington.
The individual will be expected to display good clinical skills and to work collaboratively and proactively with all teams to meet the needs of patients. This will include being actively involved in carrying out clinical skills as directed, enabling patients, their families and careers to be involved in the prevention and management of wounds
Main duties of the job
Duties include, but not limited too:
Perform holistic assessment and provide advice, care planning and treatment for patients with leg ulcers following referral from health care professionals
Plan, implement and evaluate care with patients and carer using diagnostic tests where appropriate, e.g. Doppler
Provide advice on the most appropriate wound dressing products
Provide ongoing assessment and management of venous disease
Assess for and advise re compression bandaging and hosiery
Standardise care and encourage optimal practice in care with the goal of improving patient outcomes
Encourage individualised care that enables people to tolerate treatments and thereby improve outcomes.
Agree treatment goals with the patient and a time frame for their achievement
Provide advice on management of slow healing wounds

indicated by the patients response to treatment and evaluate the overall plan
for effectiveness
Act as a resource person and provide advice,
support and education and support practice nurses, community nurses and other
health care professionals caring for patients with complex and difficult to
heal wounds

Attend educational classes in relevant areas and

for the promotion of their physical, social and psychological wellbeing
Able to work with minimal supervision in
implementing and evaluating care for patients
Ability to monitor and maintenance of stock and
equipment
To work autonomously at a level appropriate to
clinical competence and within the scope of professional practice
Priorities, organise and manage own workload in a
manner that maintains and promotes quality
To undertake with others, continuous evaluation of
this developing service to ensure it is fit for purpose and increase access and
choice
Demonstrate an ability to be flexible, organised
and function under stressful situations
Knowledgeable in the Lothian wound care formulary
