Hylton Medical Group on behalf of Sunderland West 1 Primary
Care Network is looking to employ two Advanced Nurse Practitioners to support
the Primary Care Network in the delivery of its PCN services with a focus on
same day and urgent consultations, working as part of the PCN multidisciplinary
team, delivering care within their scope of practice to the entitled patient

Main duties of the job
The Advanced Nurse Practitioner will work within their
professional boundaries. Providing care for the presenting patient from initial
history taking, clinical assessment, diagnosis, treatment and evaluation of
their care.
Working in an autonomous role
including the management of minor ailments, chronic disease, family planning,
home visits to include both acute visits and the management of chronic medical
problems in the house-bound. To provide face to face appointments, both acute and chronic
presentations, telephone-based triage, as well as support the digitisation
agenda and remote consulting.Demonstrate safe, clinical decision-making and expert care
for patients within general practice maintaining clinical excellence.
Work collaboratively with the multi-disciplinary general
practice team to meet the needs of patients, supporting the delivery of policy
and procedures. In order to work at this level, NMC requirements for advanced
practice must be met.
Develop new and innovative concepts, models, methods and
practices to deliver new and improved primary care services to meet the needs
of PCN population.
The post holder will work strictly in accordance with practice
protocols, policies and guidelines after appropriate training and assessment of
competences have been completed and satisfied.
Flexible working will be
required to meet the needs of the patients' group and the service as it
continues to develop.
Promote education and training to other staff and students.
About us
Sunderland West 1 PCN consists of 6 GP Practices covering
approx. 54,000 patients. Our network operates within the city of Sunderland.
The successful candidates will join the PCN Team and will be
supported by a senior GP as part of an MDT approach. You will have regular
development sessions, teaching training and opportunities to up-skill will be
provided and encouraged.

- Registration with health governing body (e.g., NMC, HCPC)
- Ability to manage own patient caseload
- Good communication skills
