Practice Pharmacist Full Time/Part Time Considered
Rainbow Medical Centre are looking for an enthusiastic clinical pharmacist to join our expanding team. We are a well-established large training practice with a growing list size of approximately 14,750 patients.
Our current pharmacy team comprises of 1 pharmacist, supported by 2 Medicines Management Receptionists with additional pharmacists employed by our local Primary Care Network (PCN). We also work closely with our Medicines Management colleagues. The practice clinical team also includes 10 Doctors, 10 members in our nursing team all of which are supported by a comprehensive admin/reception team.
The post holder will work alongside other clinicians to provide primary support to practice staff with regards to prescription and medication queries. They will help manage the repeat prescription system, deal with acute prescription requests, and medicines reconciliation. They will advise on safe prescribing within the practice. They will undertake medication reviews to proactively manage patients including those with complex polypharmacy.
The successful candidate will be an independent prescriber and ideally have experience of primary care.

Main duties of the job
- To provide high quality pharmacist support to the practice team members and patients in the practice.
- To deal with all suitable prescription query from community pharmacies, third party medication/supply companies and hospitals/other NHS organisations that cannot be dealt with by members of the Reception Team
- To provide support, advice and guidance to members all of the practice team.
- To review repeat medications and address concerns where appropriate
- To work with the Medicines Management Receptionist, offering guidance and support and dealing with queries, providing information as required.
- Undertaking medication reviews with patients in the practice and if required via a home visit.
- Reconciling medication on hospital letters to that contained in the patients medical record, raising queries with other clinical staff as necessary, ensuring patients receive appropriate medication.
- Reduce inappropriate poly-pharmacy and wasteful prescribing through medication review

Rainbow prides itself on its practice team, comprising of clinicians and non clinicians who work well together to provide the best care for the registered population. The practice has a low staff turnover but a young partnership (due to the retirement of the previous partners). The practice is keen to find an Pharmacist who wants to become part of the team.
The practice holds regular weekly meetings which cover clinical and non-clinical changes/developments within the practice, local area and nationally. The postholder will attend the meetings and be fully involved in them.
