Rainbow Medical Centre has created an additional Advanced Nurse Practitioner (ANP) role, as part of our ongoing commitment to expanding and enhancing the services we provide to our patients. This is an excellent opportunity for an experienced ANP to join a well-established large training practice with a growing list size of approximately 14,750 patients.

Our clinical team include 10 Doctors, 1 ANP, 9 other members of our nursing team (including a Nurse Manager, specialist nurses and HCAs) all supported by a comprehensive admin/reception team.

We envisage this post being a hybrid post; the successful candidate will be primarily responsible for managing our home-visiting service both acute presentations and also chronic disease/frailty reviews. There will also be some opportunity to see acute patients within the surgery.

The post will include working autonomously to deliver care to our patients with a particular focus on patients who are on the frailty register and/or requiring a home visit.

The successful candidate will be an independent prescriber (or near completion of their prescribing training), ideally with general practice/frailty experience and would enjoy a position that works both in the practice but also patients own homes.

Main duties of the job

Main duties of the role please :

  • As an autonomous practitioner with a prescribing role, the post-holder is responsible for the care they deliver. They will demonstrate advanced clinical expertise, critical thinking and clinical decision-making.
  • The post holder will exercise independent prescribing rights in accordance with current legislation and NMC guidance.
  • The focus of the role is on the delivery of evidence-based care for patients with acute, non-acute and Long-term conditions with a particular focus on those on the frailty register and patients who require home visits.
  • Working independently, the post holder will provide care for the presenting patient from initial history taking through to clinical assessment, diagnosis, treatment and evaluation of care; working within their own competencies, seeking advice or referring to colleagues when appropriate.
  • The post holder will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of practice policy and procedures.

Rainbow Medical Centre is a long established training Practice, with premises in Sutton and Thatto Heath and a registered population of 14,750 patients. Rated 'Good' overall by the CQC and high QOF achieving.Rainbow prides itself on its practice team, comprising of clinicians and non clinicians who work well together to provide the best care for the registered population. The practice has a low staff turnover but a young partnership (due to the retirement of the previous partners). The practice is keen to find an ANP who wants to become part of the team.The practice holds regular weekly meetings which cover clinical and non-clinical changes/developments within the practice, local area and nationally. Clinical practice

Support patients to adopt health promotion strategies and apply principles of self-care

Support and manage health needs of women presenting for family planning and cervical cytology consultations

Where appropriate, implement and participate in vaccination and immunisation programmes for both adults and children

Demonstrate sensitive communication styles to ensure patients and carers are fully informed and consent to treatment, using alternative methods of communication to overcome different levels of understanding and cultural background etc.

Develops and delivers presentations to all members and levels of MDT both within the practice and to the wider CCG/regional/national community promoting good practice and innovation.
