An opportunity has arisen for an experienced Advanced Nurse Practitioner to join our team.
We have a great team of clinical and clerical staff and just need one more to make us complete - could it be you?
Main duties of the job
You will be responsible for seeing up to 30 patients a day and for managing their care.
No two days are the same.
We would like the applicant to be able to confidently undertake a "Duty Clinic" role once a week whereby you see emergency presentations and deal with on the day situations including the issuing of prescriptions and speaking to community partners with confidence such as NWAS and our Macmillan and Community Nurse colleagues.

The following are the core
responsibilities of the ANP.
There may be on occasion a
requirement to carry out other tasks; this will be dependent upon factors such
as workload and staffing levels:

responsibilities:
- Undertake a
clinical duty role once a week as per the clinical rota and provide cover to other clinical staff performing this role in case of sickness/holiday
- Provide a
triage service, actively managing or referring patients or treating patients
within own scope of practice
- Carry out
home visits if required as per clinical remit & own scope of practice and following your agreement
- Support the
clinical team in the development of individual treatment plans for chronic
disease patients.
- Identify,
manage and support patients at risk of developing long-term conditions,
preventing adverse effects on the patients health
- Carry out
medical audits
- Complete own
prescribing and some limited EPS prescriptions
- Action limited correspondence via Docman
- Develop,
implement & embed health promotion and well-being programmes
- Provide
routine and acute nursing care to patients as required in accordance with
clinical based evidence, NICE and the NSF
- Request
pathology services as necessary, processing and interpreting results as
required
- Refer
patients directly to other services as appropriate
- Maintain
accurate clinical records in conjunction with extant legislation
- Ensure
appropriate read-codes are used effectively
- Support the
team in dealing within clinical emergencies
- Liaise with
external services/agencies to ensure the patient is supported appropriately
(vulnerable patients etc)
- Delegate
clinical responsibilities appropriately (ensure safe practice and the task is
within the scope of practice of the individual
