Job summary

The long-established Rycote Partnership of Thame are excited to be taking on Chiltern and Brook surgeries in Watlington & Chalgrove. We are looking to expand clinical cover and are seeking to appoint advanced nurse practitioners in these surgeries to complement the three that are already based in Thame. You will join an expanded GP team alongside existing paramedic, pharmacists and social prescribers to strengthen the multi-disciplinary team. This is an excellent opportunity for an experienced nurse practitioner to utilise and develop their clinical skills in a supportive and friendly environment.

Main duties of the job

The successful applicant will ideally be an experienced nurse practitioner with a prescribing qualification. The post holder will enjoy a varied role which combines seeing acute conditions with long term chronic disease management. Additional training in chronic disease management will be available for the right candidate.

The successful applicant will:

  • Autonomously treat patients with undifferentiated and undiagnosed conditions whilst having an awareness of when to appropriately access the wider clinical team
  • Manage patients with long term chronic diseases through proactive and reactive interventions
  • Hold an extended/supplementary nurse prescribing qualification and have experience of using it to treat a range of conditions
  • Be capable and comfortable with face to face, telephone and e-consulting (as well as home visiting where appropriate)
  • Possess the organisational and interpersonal skills to manage expectation and practice safely in a fast-paced environment

The partnership of the new Rycote Group comprises 5 clinical partners and a managing partner.   • To deliver a high standard of patient care as an Advanced Nurse Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth evidence-based approach.
  • To work as a clinical autonomous independent practitioner to assess, diagnose and treat patients with a range of acute, non acute and chronic medical conditions within the scope of training and accreditation.
  • Provide a range of face to face, telephone and online consultations as appropriate to the patient and their condition.
  • Work with GPs, nurses and the wider multi-disciplinary team to deliver the highest possible care to patients within the resources available.

Job Responsibilities:

Clinical Care

  • Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.
  • Clinically examine and assess the patient instigating necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports and plan clinical care accordingly.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • To prescribe safe, evidence based, cost effective and appropriate medication in accordance with current legislative framework and within the post holders training and registration, to include advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies
  • Work with patients and their carers in order to support compliance with and adherence to prescribed treatments
