Job summary
We are seeking a professional and well-organised Care Coordinator to join North Birmingham Primary Care Network (PCN) under the NHS DES contract. This role is based solely at Shah Zaman Surgery and offers 12 hours per week across 3 days.
The successful candidate will support the practices safeguarding responsibilities and manage patient recall processesplaying a key role in ensuring patients receive timely, coordinated, and appropriate care. This is a vital position that contributes directly to improving health outcomes within our local community.
You will work closely with clinical and administrative teams to streamline care pathways, enhance communication, and support the delivery of high-quality, patient-centred services. Main duties of the job
The Care Coordinator will support the delivery of personalised, coordinated care for patients at Shah Zaman Surgery. This includes managing patient recalls and playing a key role in safeguarding processes. The post holder will arrange safeguarding meetings, take accurate minutes, and maintain the safeguarding register in collaboration with the practices Safeguarding Lead.
They will coordinate care plans, support shared decision-making, and help patients navigate services across health and social care. The role involves proactive communication with patients, managing appointments, and ensuring access to clear information to support self-management and informed choices.
Working closely with clinical teams, social prescribing link workers, and other professionals, the Care Coordinator will contribute to multidisciplinary team discussions and ensure safeguarding concerns are escalated appropriately. This non-clinical role requires strong organisational skills, attention to detail, and a commitment to improving patient outcomes through personalised, proactive care.

Contact patients, book appointments, and ensure accurate documentation of recall activity.
Administrative SupportAssist the administrative team with answering incoming telephone calls and responding to patient queries.
Provide general administrative support as needed to ensure the smooth running of the practice.
Support reception and back-office functions during busy periods or staff absence.
Communication and NavigationAct as a point of contact for patients and carers, helping them navigate health and care services.
Provide high-quality written and verbal information to support patient understanding and decision-making.
Liaise with internal and external services to ensure continuity of care.
Multidisciplinary Team (MDT) SupportSupport the coordination and delivery of MDT meetings.
Contribute to case discussions and ensure actions are followed up.
Work collaboratively with GPs, nurses, social prescribing link workers, and other professionals.
Training and DevelopmentUndertake or be enrolled in appropriate training as outlined by the Personalised Care Institute and the Workforce Development Framework for Care Coordinators.
Participate in supervision and ongoing professional development.
