Job summary

Alvaston Medical Centre is looking for an Advanced Clinical Practitioner Nurse to join our Practice.

The Advanced Nurse Practitioner is part of a clinical team providing face to face consultations and home visits to patients in a primary care setting. The Advanced Nurse Practitioner is a professionally autonomous practitioner managing undiagnosed problems using advanced nursing skills. The post holder will assist in developing protocols and guidelines within the service and work with clinical colleagues in ensuring the highest standards of care for all patients.

Main duties of the job

The purpose of role is to assist the clinical team in the service and delivery of quality care management for the practice population.

Clinical Responsibilities:To use advanced practice skills within consultation to assess, diagnose, advise, treat and refer patients both in Surgery and on Home visits,To provide a comprehensive assessment,Arranging and interpreting appropriate investigations,Develop, implement and embed health promotion and well-being programmes,Support the clinical team in the development of individual treatment plans for chronic disease patients,Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health,Ensure read codes are used effectively,Prioritise health issues and intervene appropriately,Support the team in dealing with clinical emergencies,Recognise, assess and refer patients presenting with mental health needs,Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways,Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.),Support the clinical team with all safeguarding matters, in accordance with local and national policies,Deliver opportunistic health promotion where appropriate

About us

We have a team of 40+ working at Alvaston and our branch site in Aston-on-Trent. We have a strong clinical team consisting of 3 partners GP's, Physician Associates, ANP's, Nurses and HCA's and as a dispensing practice we have a team of Clinical Pharmacists on site.

We have an external team which support consisting of a Physio, Midwife, Social Prescribers and Care Co-ordinators and we work closely with all of our third party partners.

Of our 11,600 patient base, we prescribe to 3,000 of these.

We are a friendly and supportive team, why not come and visit us and see for yourself? Our clinical and admin teams worked closely together to deliver the best service we can.

As a teaching practice we have registrars and students on site throughout the year.

We are easily accessible from the M1, A50 and A52 and a number of our team are based in Leicestershire and Nottinghamshire.

We have a strong PPG and they support us in gaining patient feedback and help us in identifying opportunities to improve our service.

Operate under appropriate clinical supervision of a named GP supervisor.

Provide first point of contact care for patients presenting with undifferentiated and/or undiagnosed problems, utilising history-taking, physical examinations, and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient (and their carers where applicable).

Recognise and work within own competence and professional code of conduct as regulated by the NMC and RCN General Practice Advanced Nurse Practitioner Competencies (2018)

Undertake face-to-face, telephone and online consultations for emergency or routine problems, as determined by the Practice, including management of patients with long-term conditions.

Participate in all treatment and preventative healthcare services in the Practice, as delegated and agreed by supervising GP's.

Perform diagnostic physical examinations and treatment.

Identify, signpost, or refer patients at risk of developing long-term conditions, preventing adverse effects on patients' health.

Organise onward investigation, treatment and referral where appropriate.

Ensure continuity of care, arranging follow-up consultations or reviews as necessary.

Attend regular Practice clinical meetings.

Support clinical team with all safeguarding matters in accordance with local and national policies.

Under supervision of Practice GP's, make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the Practice.

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

Review and act upon laboratory results and clinical communications from hospitals and other providers.

Undertake complex clinical coding.

Recognise, assess and refer where appropriate, patients presenting with mental health problems.

Produce care plans for frequent attendees and undertake initial consultations.

Understand Practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.

Oversee referral process and intervene where appropriate.

Contribute to administrative element of clinical reports and initial observations, prior to GP appointment.

Be able to justify choice of medication and support patients in use of their prescribed medication (within own scope of practice).

Be able to understand impact of co-morbidities, other medications and polypharmacy.

Deliver integrated patient-centred care through appropriate working with the wider multi-disciplinary team and social care network

Conduct home visits and care home visits as necessary (follow adequate training).

Act on alerts and reminders during consultations (e.g. QOF, local enhanced services etc.).
