Job summary

The PCN Nurse is an autonomous practitioner responsible for delivering high-quality, proactive, and complex nursing care to a defined population across the Primary Care Network. The role focuses on managing patients with multiple long-term conditions and comorbidities, preventing deterioration, and supporting admission avoidance through advanced clinical assessment, decision-making, and care planning. Working closely with the Community Paramedic, GPs, District Nursing, and the wider MDT, the PCN Nurse contributes to population health priorities, and supports safe, integrated care across primary and community services, in line with professional, governance, and quality standards.

Main duties of the job

Key Responsibilities

  • Manage a defined PCN population caseload, delivering proactive, preventative, time-limited and complex nursing care
  • Undertake autonomous, holistic assessments and advanced clinical decision-making within scope of practice
  • Deliver specialised clinical care including chronic disease management, wound and pressure area care, venepuncture, vaccinations, ear irrigation, and palliative and end-of-life care
  • Identify early clinical deterioration, unmet need, and cognitive decline, using risk stratification and population health approaches
  • Implement admission avoidance interventions and escalate appropriately within the MDT
  • Work collaboratively with the Community Paramedic and wider MDT to support integrated care
  • Refer patients requiring ongoing, intensive nursing care to District Nursing services in line with agreed pathways
  • Maintain accurate and timely clinical records in line with professional standards and governance frameworks
  • Participate in audit, quality improvement, mandatory training, appraisal, and service development across the PCN

Medway Rainham Primary Care Network (PCN) is seeking a skilled and motivated PCN Nurseto deliver proactive, preventative and time-limited nursing care across the network. Working across GP practices and through home visiting, the role focuses on early identification of clinical deterioration, admission avoidance and supporting patients to remain well, independent and safely managed.

The post-holder will manage a defined PCN population caseload, using risk stratification and population health approaches to identify patients who would benefit from early intervention and coordinated care. This role supports patients before they require long-term or intensive District Nursing input and does not replace or duplicate existing community nursing services.

You will work autonomously within your professional scope of practice, undertaking comprehensive nursing assessments, clinical decision-making and care planning, while collaborating closely with the Community Paramedic and wider multidisciplinary team. Clear escalation pathways and strong PCN clinical governance arrangements are in place to support safe, effective practice.

The PCN Nurse plays a key role in delivering integrated, patient-centred care, contributing to service development, quality improvement and preventative health initiatives across the PCN.
