Job summary
The PCN Nurse is an autonomous practitioner responsible for delivering high-quality, proactive, and complex nursing care to a defined population across the Primary Care Network. The role focuses on managing patients with multiple long-term conditions and comorbidities, preventing deterioration, and supporting admission avoidance through advanced clinical assessment, decision-making, and care planning. Working closely with the Community Paramedic, GPs, District Nursing, and the wider MDT, the PCN Nurse contributes to population health priorities, and supports safe, integrated care across primary and community services, in line with professional, governance, and quality standards.
Main duties of the job
Key Responsibilities
- Manage a defined PCN population caseload, delivering proactive, preventative, time-limited and complex nursing care
- Undertake autonomous, holistic assessments and advanced clinical decision-making within scope of practice
- Deliver specialised clinical care including chronic disease management, wound and pressure area care, venepuncture, vaccinations, ear irrigation, and palliative and end-of-life care
- Identify early clinical deterioration, unmet need, and cognitive decline, using risk stratification and population health approaches
- Implement admission avoidance interventions and escalate appropriately within the MDT
- Work collaboratively with the Community Paramedic and wider MDT to support integrated care
- Refer patients requiring ongoing, intensive nursing care to District Nursing services in line with agreed pathways
- Maintain accurate and timely clinical records in line with professional standards and governance frameworks
- Participate in audit, quality improvement, mandatory training, appraisal, and service development across the PCN
