Job summary

We are looking for a committed, enthusiastic Practice Nurse to join our friendly practice in Grimsby with approx. 2600 patients. We currently have 1 GP, 1 ANP/ACP 1 Practice Nurse and a Health Care Assistant. We are a SystmOne Practice and we achieve high QOF scores.

You will be able to demonstrate an ability to work efficiently and autonomously. Whilst being patient and team centered. Your duties will include Chronic Disease Management, Cytology, Travel Health and Child Immunisation, Family Planning and Wound Management is also desirable. Further Training will be given to the successful applicant with a possibility of training to ACP.

The post holder is responsible for the delivery of basic practice nursing services care to the practice patient population. Supported by the clinical team, they will deliver care within the boundaries of their role. Focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required

Main duties of the job

Develop and implement programmes to promote health and well-being

Implement and evaluate individual treatment plans for patients at risk of developing long term conditions

Prioritise health problems and intervene appropriately to assist the patient in complex, or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self-care

Provide information and advice on medications or medication regimens

Support patients to adopt strategies that promote patients to live healthily and encourage self-care

Assess and care for patients presenting with uncomplicated wounds

Support and manage health needs of women presenting for cervical cytology and gyneacology concerns

We are a small friendly GP surgery located in a mutli-purpose building in Grimsby, near the town centre.

We are looking for an enthusiastic Practice Nurse to join our small team. To work collaboratively with our Clinical Team and help prioritise all aspects of patient care. To deliver care within the boundaries of their role focusing on Chronic Disease Management.
