The Post Holder will manage a caseload and deal with a wide range of health needs, within a small GP surgeryensuring the highest standards of care for all patients.
The ANP/ACP will work within the team to provide high quality care which meets the practices diverse needs

About us
We are a small GP Sugary located within a multi function building in Cromwell Primary Care Centre in Grimsby.
We have a small clinical and admin team who work as a tight knitted unit so the successful applicant with need to work autonomously and join the team.

Job responsibilities
- Assess/examine patients, diagnose, plan, treat and review/evaluate. Assessment may be face-to-face by appointment or by telephone as part of a triage/unscheduled care team and will include patients presenting with an undifferentiated diagnosis.
- Diagnose and manage patients presenting with both acute and chronic conditions based on a management plan which includes a medication plan and other interventions.
- Identify health priorities and make timely and appropriate interventions to support patients presenting in an emergency and/or with complex/urgent needs.
- Assess and manage patients physiological and psychological needs, including patients with complex needs, multiple diagnoses and polypharmacy.
- In line with competence and agreed protocols, prescribe medication within local formulary and make appropriate adjustments to treatment, ensuring patient understanding of medication side-effects and interactions, including over-the-counter medication.
- Ensure the team meets the clinical needs of patients presenting with routine and emergency needs and conditions or who require advice and support including chronic disease management, family planning, vaccination and immunisations, health promotion/lifestyle advice, wound care, etc.
- Ensure that all patients who may be vulnerable or who have mental health needs are appropriately managed and refer on to the necessary mental health team. Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
- Assess the healthcare needs of patients with undifferentiated and undiagnosed problems, order and interpret diagnostics as clinically indicated
- Prescribe in accordance with the Centre prescribing formulary (or generically) whenever this is clinically appropriate and engage with the medicines management strategy for the Centre

- Be able to introduce review and update clinical guidelines and protocols in accordance with latest evidence base

- Offer guidance and support within the team including ensuring and maintaining individuals competence
- Implement Risk Assessments to identify hazards and minimize risks to others
Desirable
- Lead and contribute to in house learning and training events
- Assist in dealing with clinical governance activities and clinical audits to ensure continual improvements of service delivery
- Be able to identify opportunities for shared learning

- Take the lead in clinical performance review process
