Job summary

To provide care for the presenting patient with a minor illness or injury, taking initial history, clinical assessment, diagnosis, treatment and evaluation of care. Patients will have booked appointments at 15 minute intervals. There may be a requirement to see extra or emergency patients when necessary. The Nurse Practitioner will prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner. The Nurse Practitioner will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Main duties of the job

To work autonomously seeing patients with undifferentiated problems, setting and maintaining patient care standards.

To assess, diagnose and treat patients presenting with a wide range of clinical conditions, both in complexity and severity.

To operate as a generalist providing complete episodes of care for patients of any age and with a wide variety of presenting problems and health care needs.

Assist with completion of Annual and periodic Long Tearm Health Reviews as per QoF requirements

Main Responsibilities:

Clinically examine and assess patient need and plan clinical care accordingly.

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.

Help practice in completion of all annual reviews of Long Term conditions as per QoF requirements and protocols.

Prescribe and review medication for therapeutic effectiveness.

Work with patients in order to support compliance with and adherence to prescribed treatments.

Provide information and advice on prescribed or over-the-counter medication on medication regimes, side effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies to promote healthy lifestyles and apply principles of self-care.

Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.

Refer patients to other members of the primary and secondary health care teams as appropriate

Ensure clinical practice is safe and effective and remains within boundaries of competence and acknowledge limitations
