Healthcare Support Worker

Job summary

We are seeking an enthusiastic HCA, HWBC or Healthcare Support Worker to expand our home visiting team. With a special interest in frailty and older people, supporting delivery of same day care in the community.

The successful candidate will work with our existing team to visit patients home to assist with Acute and ongoing health health concerns.

Main duties of the job

The following are the core responsibilities of the home visiting role:

  • Carry out baseline observations such as pulse oximetry, blood pressure, temperature, and pulse rate, recording findings accurately
  • Facilitate routine blood pressure monitoring and advising patients accordingly
  • Undertake wound care, dressings and other clinical tasks
  • Support the practice clinicians with the management of chronic disease clinics
  • Support the practice nurse with health promotion programmes
  • Carry out BMI checks as directed
  • Act as a chaperone as required
  • When trained, administer flu vaccinations
  • Ensure specimens are recorded and ready for onward transportation
  • Deliver opportunistic health promotion where appropriate
  • Be prepared to work across Plymouth to support our 5 locations, this will be communicated at short notice due to acute patients health concerns

We are a dynamic, friendly and supportive PCN with much experience in training GPs and other healthcare professionals. Sound PCN have been early adopters of new models of NHS care, utilising the diverse skill mix offered by physiotherapists, paramedic, nurse practitioners, social prescribers, pharmacy technicians and care co-ordinators.

This role is to support our member practices as part of our home visiting community care team, working during the working day, Monday to Friday.

To work without direct supervision within the clearly defined boundaries of your competencies &carry out specific delegated clinical tasks/responsibilities.

To take a holistic approach to patient management, carry out tasks/treatment plans as directed by registered staff, ensuring to report back to the appropriate worker any changes in clinical conditions &maintain accurate clinical records.

To provide clinical/clerical duties relevant to the service you work within.

To participate in all mandatory training required for the role & participate in training that is role specific.

You will be required to travel to patients based across Plymouth

The postholder must be able to refer and/or signpost patients in their own home or care home to other allied health professionals for example community COPD nurses, Social prescribers etc.

Qualifications

Essential

  • Maths and English GCSE grade 4 or above (A-C).
  • NVQ III in Care OR able to demonstrate the skills knowledge & ability to work to this level.

Desirable

  • Qualified in Phlebotomy

Experience

Essential

  • Experience with working within a health care setting

Desirable

  • Experience of dealing with vulnerable patients

Skills

Essential

  • Valid UK driving license and ability to travel across Plymouth.
  • Basic IT skills
  • Ability to use initiative and work independently
  • Good time management
