Job summary

We are seeking an enthusiastic Nurse, Paramedic, OT or Physiotherapist to expand our home visiting team. A clinician with a special interest in frailty and older people, supporting delivery of same day care in the community.

Main duties of the job

Main duties

a) Undertake same day urgent visits, prioritising in order of clinical need, acknowledging this can change at short notice due to service demand.

b) Undertake comprehensive geriatric assessments including medication reviews and advanced care planning where appropriate.

Take an accurate history, undertake suitable systems examination including observations and other point of care tests as appropriate.

Diagnose and manage acute and chronic conditions.

Utilise and refer to community services where appropriate, ensuring patient safety throughout.

Communicate findings and management plan with patient/relatives and referring clinician.

Provide safety netting/ worsening advice for patients and relatives.

Ensure comprehensive, timely and accurate documentation.

This role is for a clinician to support primary care as part of our community care team working during the working day, Monday to Friday.

The post holder will provide holistic care for individuals in their homes including assessment, diagnosis and management to provide the best outcomes for the patient.

The post holder will have the ability and confidence to safely and comprehensively carry out a full systems assessment, autonomously discharging patients within your own scope of practice seeking senior colleague advice as required. It is important that you are comfortable working in this capacity though you will always have access to support from senior clinical colleagues at other sites.

Essential

  • Regulated Health Care professional with current registration: (NMC/HCPC Registered).
  • Minimum of three years post registration with demonstrable experience of using diagnostic skills
  • Postgraduate certificate or diploma (nurses only)

Desirable

  • Recognised qualification in physical assessment and clinical reasoning or equivalent.

Skills

  • Be able to competently assess and examine all clinical systems and interpret and communicate findings accurately and succinctly.
  • Access to own transport and ability to travel across the locality on a daily basis.
  • Ability to collect and record information and data, for record- keeping, monitoring and evaluation.
  • Ability to work safely and autonomously in home settings.
  • Assess and manage clinical risk and diagnostic uncertainty.
  • Maintain personal and patient safety during visits.
  • Demonstrate good time management skills.
