Job summary
We are looking for an experienced Practcie Nurse to join our team. This is an additional role to help deliver safe and effective nursing care to the Practice population at our sites in Poringland and Rockland St Mary. We are investing in our Nursing team.

The successful candidate would be joining our existing nursing team which includes a Nurse Practitioner, Practice Nurses, a Nursing Associate and Health Care Assistants.
Focusing on the responsibilities of the role, the Nurse would work with the wider Practice team to deliver national, local and Practice healthcare priorities.
We are seeking candidates who have experience in General Practice. Perhaps someone who is looking for a change or themselves has a change in circumstances.
The Practice Nurse role is now key in the management of long term conditions and health promotion. We have extended the face to face clinical time spent with patients to take a holistic approach to their well-being.
Our Practice team work well together and a recent staff survey reported to the Partners that our staff are proud to work at the Practice and enjoy working at Heathgate. This was an encouraging response to the questionnaire and we hope the successful candidate will feel the same when they join us.

Main duties of the job
These are the main clinical responsibilities:
- Provide routine nursing care to patients in line with clinical based protocols, guidance and evidence.
- Support patients with the management of their long term conditions via regular review, agreement of treatment plans and follow up.
- To identify patients at risk of developing long term conditions.
- To provide wound care to patients.
- To provide travel medicine assessments
- To implement national and local vaccination programmes for adults and children.
- To support the 'same day need' team within the Practice with medical emergencies.
- To support and deliver national screening programmes such as the cervical screening and NHS Health check programmes.
- With HCA's undertake phlebotomy services.
- To undertake a range of investigations for clinical assessment such as ECGs, spirometry and audiometry.
- To liaise and refer patients to other providers of health care and health promotion services.
- To delegate clinical responsibilities to members of the nursing team as appropriate and necessary, where they are trained to deliver such.
- To support local safeguarding policies - both adult and child.
- To provide sexual health services including contraception advice and checks.
- To provide treatment room duties such as the removal of sutures and post operative wound care.

Heathgate Medical Practice is offering General Medical Services to the patients in an eighty square mile semi rural part of South Norfolk bordering Norwich. We have our main site in Poringland and a branch site in the rural village of Rockland St Mary.
We have a strong, loyal and committed group of clinical and non-clinical team players who support our 10,000 patients.
Our team is enhanced by services offered by our Primary Care Network including first contact physiotherapists, social prescriber, mental health care practitioner, pharmacist and pharmacy technician.
The services we provide are traditionally akin to a GP Practice, but with regular review, has seen changes and developments in the way we manage our services and offer these to our patients.
We have developed our digital offer as well as the more traditional type of services extended through face to face appointments with our clinical team.
A strong management team supports the delivery of our services which patients talk positively about in both national and local surveys. Healthwatch Norfolk also gauge the opinions of our patients through their regular engagement exercise.
We do not promote the 'best' service for our patients, we advocate delivery of the 'appropriate' service through the Practcie values which underpin our actions. These are to be, caring, flexible, professional, effective and have integrity in all we do.
