Job responsibilities
The post holder will be
responsible for ensuring the delivery of safe and effective nursing care to the
Practice population at our sites in Poringland and Rockland St Mary.
As our nurse lead, the incumbent
will manage and support the team with day-to-day nursing responsibilities and lead
on the development of the team, working with the Partners and Management to
deliver national, local and Practice healthcare priorities.
Clinically, the focus of the
role is the delivery of care for patients with long term conditions, health promotion
and screening. As an autonomous
practitioner, the nurse is responsible for the care delivered, demonstrating
critical thinking skills in clinical decision making.
Job responsibilities(Clinical)
To provide routine nursing
care to patients in accordance with clinical based evidence.
To support patients with the
management of their long-term conditions via regular review, agreeing treatment
plans in line with national guidance for the management of the condition.
To identify patients at risk
of developing long term conditions and support with management plans.
To provide wound care to
patients.
To provide travel medicine
assessments.
To implement NHS vaccination
programmes for adults and children.
To support the team with medical
emergencies within the Practice.
To support and deliver
national screening programmes such as cervical screening.
To request pathology
services as necessary.
To undertake a range of
investigations for clinical assessment such as ECGs, audiometry, and spirometry.
To liaise and refer patients
to other providers of health care and health promotion services.
To delegate clinical
responsibilities to members of the nursing team as appropriate and necessary,
where they are trained to deliver such.
To support local safeguarding
policies both adult and child.
To provide treatment room
duties such as the removal of sutures and post operative wound care.
Some of these responsibilities
are primarily undertaken by the Health Care Assistants but with the Practice
Nurse supporting where necessary to manage demand.
Other clinical tasks may
include:
Maintaining and cleaning equipment
used in the Practice.
Maintaining stock used by
the clinical team.
Ordering supplies including
vaccinations to maintain stock at a level to deliver the appropriate
care/service.
Chaperoning and assisting
patients where appropriate who are being examined by other clinicians.
Other delegated duties appropriate
to the post.
Job responsibilities (Leadership)
Act as a clinical leader in
the delivery of nursing services to patients.
Lead the nursing team in the
planning and implementation of local and national guidance (including Practice
protocol and policy).
Promote the role of the
nursing team in the provision of care.
Participate in the weekly
Business Meeting, promoting the nursing agenda.
Lead nurse team meetings on
a regular basis.
Promoting a learning environment
within the nursing team with a no blame culture.
Establish and lead a programme
of clinical supervision of the nursing team, reviewing their practise with them
via observations at agreed intervals.
Support with the Practcie
strategy for the delivery of national target domains such as the Quality and
Outcomes Framework.
Lead the team to make
realistic self-assessments of their knowledge via regular review and appraisal,
challenging complacency and encouraging development and learning opportunities.
Take responsibility for
their own development and learning, acting as a positive role model for the
nursing team.
Lead on infection prevention
and control, with regular audit and review of process and protocol.
Create and implement a nurse
audit programme for the Practice.
Team working
Work as an
effective and responsible team leader, supporting others and exploring the
mechanisms to develop new and creative ways of working.
Lead team
activities that create opportunities to improve both patient care and staff
well-being.
Delegate
appropriately, adopting the principles of safe practice and assessment of
competency of the nursing team.
Prioritise
workload within the nursing team and ensure effective time-management
strategies are embedded within the culture of the team.
Management of risk
Manage and
assess risk within the areas of responsibility within the role, ensuring
adequate measures are in place to protect staff and patients.
Monitor work
areas to ensure they are safe and free from hazards and confirm to health,
safety and security legislation, policies and guidelines.
Act as a
role model to support members of the nursing team to undertake mandatory and
statutory training requirements.
Apply
infection control measures within the Practice according to national and local
guidelines.

In
the course of seeking treatment, patients entrust us with, or allow us to
gather, sensitive information in relation to their health and other
matters.
They
do so in confidence and have the right to expect that staff will respect their
privacy and act appropriately.
In
the performance of the duties outlined in this Job Description, the post-holder
may have access to confidential information relating to patients and their
carers, practice staff and other healthcare workers. They may also have access to information
relating to the practice as a business organisation. All such information
from any source is to be regarded as strictly confidential.
Information
relating to patients, carers, colleagues, other healthcare workers or the business
of the practice may only be divulged to authorised persons in accordance with
the practice policies and procedures relating to confidentiality and the
protection of personal and sensitive data.

The post
holder will be required to keep up to date with mandatory and statutory training

Professional registration
Registration
with the Nursing and Midwifery Council is a requirement of the post including compliance
with the new regulations regarding re-validation
