Job summary

The post holder will be responsible for ensuring the delivery of safe and effective nursing care to the Practice population at our sites in Poringland and Rockland St Mary.

As our nurse lead, the incumbent will manage and support the team with day-to-day nursing responsibilities and lead on the development of the team, working with the Partners and Management to deliver national, local and Practice healthcare priorities.

Clinically, the focus of the role is the delivery of care for patients with long term conditions, health promotion and screening. As an autonomous practitioner, the nurse is responsible for the care delivered, demonstrating critical thinking skills in clinical decision making.

This role is open to nurses who have a background of working in Primary Care.

Main duties of the job

There are two parts to this role. A clinical responsibility (60%) and leadership (40%).

The clinical aspects are to provide routine nursing care to patients in accordance with clinical protocols, national and local guidance, supporting patients with the management of their long term conditions via regular review and agreeing treatment plans.

To implement NHS vaccination and screening programmes and provide a range of investigations for clinical assessment by the wider Practcie team.

To provide treatment room duties and support local safeguarding policies.

In terms of leadership, we are looking for the incumbent to act as a clinical leader in the delivery of nursing services to patients and the nursing team in planning and implementation of local and national guidance.

They will promote the role of the nursing team in the provision of care, participating in the weekly business meeting promoting the nursing agenda.

The nurse lead will facilitate regular team meetings, promote a learning environment within the team with a no blame culture.

Support the Practcie strategy for the delivery of national target domains such as the Quality and Outcomes Framework and IIF.

Leading the team to make realistic self-assessments of their knowledge via regular review and appraisal, challenging complacency and encouraging development and learning opportunities.

Create and implement a nurse audit programme for the Practice.

To provide wound care to patients.

To provide travel medicine assessments.

To implement NHS vaccination programmes for adults and children.

To support the team with medical emergencies within the Practice.

To support and deliver national screening programmes such as cervical screening.

To request pathology services as necessary.

To undertake a range of investigations for clinical assessment such as ECGs, audiometry, and spirometry.

To liaise and refer patients to other providers of health care and health promotion services.

To delegate clinical responsibilities to members of the nursing team as appropriate and necessary, where they are trained to deliver such.

To support local safeguarding policies both adult and child.

To provide treatment room duties such as the removal of sutures and post operative wound care.

Some of these responsibilities are primarily undertaken by the Health Care Assistants but with the Practice Nurse supporting where necessary to manage demand.

Other clinical tasks may include:

Maintaining and cleaning equipment used in the Practice.

Maintaining stock used by the clinical team.

Ordering supplies including vaccinations to maintain stock at a level to deliver the appropriate care/service.

Chaperoning and assisting patients where appropriate who are being examined by other clinicians.

Other delegated duties appropriate to the post.
