Job summary
We are looking for an experienced Practice Nurse specialising in Chronic Disease, who likes a challenge and who can work autonomously
within the Practice using all aspects of their Practice skillsets, to provide holistic care to our Practice population.
The successful candidate would be expected to have experience of dealing with Chronic Disease patients and be flexible to cover general aspects of practice nursing.
The expectation is to run clinics within the guidelines of protocols for the wellbeing of patients on Chronic Disease registers. Hold or
studying towards an appropriate qualification to effectively run such clinics.
General Practice Nurses are health professionals who
have the capability to make sound judgements and will be expected to advise
patients on general health care, health promotion, illness prevention, minor
ailments, acting as counsellor and adviser as required, with referral to GP as
necessary.
Main duties of the job

nursing treatment to patients in participation with General Practitioners or
independently within agreed protocols.
Provide
general and specific health screening to the practice patients within agreed
protocols with referral to GP as necessary.
Advise
patients on general health care, health promotion, illness prevention, minor
ailments, acting as counsellor and adviser as required, with referral to GP as
necessary.
Advise
patients on test results and liaise with GP for prescriptions as necessary.
Provide
telephone advice to patients as required.
Undertake
diagnostic tests blood and urine tests, ECGs, carry out test and follow ups
as necessary. Discuss results with
patients as necessary.
Treatment
of minor injuries, dressings, removal of sutures, routine injections,
immunisations as required.

blood samples, swabs, smears etc.
