An exciting opportunity has arisen for an experienced, committed Advanced Nurse Practitioner to join our group of GP practices in North Camden, Hampstead Heath.

ThisAdvanced Nurse Practitionerrole will involve working alongside a multidisciplinary team, including GPs, Clinical Pharmacists, Nurses and HCAs to provide care and treatment to patients with long term conditions and undiagnosed undifferentiated diagnoses.

The successfulAdvanced Nurse Practitionerwill work as an autonomous practitioner, diagnosing, managing and discharging patients with illness or injury, as well as providing specialist clinics such as smears, dressings and child immunisations and will be fully supported by their primary care colleagues.

Main duties of the job

What we are looking for in an Advanced Nurse Practitioner;

Registered Adult Nurse registered with the Nursing & Midwifery Council (NMC),

Hold anAdvanced Nurse Practitionerqualification

Can show evidence of using autonomous physical examination and assessment skills in practice

Can demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children

Knowledge and experience of QOF

About us

The team you will be working with as an Advanced Nurse Practitioner;

A fully staffed multi-disciplinary team including, GPs, Registrars, Physician Associates, HCAs, and a full complement of Administration Staff

Across the 5 practices in the vicinity, you will be supported by over 20 GPs and 8 Practice Nurses

You will be based specifically at Park End Surgery, however you may be required to work across or on behalf of the other practices in the area.

Competitive salary; £48,000 - £60,000 per annum pro rota

Job description

Job responsibilities

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

b. Implement and evaluate individual specialised treatment plans for chronic disease patients

c. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

d. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

e. Provide wound care (ulcer/Doppler, etc.)

f. Provide specialist clinics such as Smear, Child Imms, Ear syringing, contraception, etc.

g. Develop, implement and embed health promotion and wellbeing programmes

h. Undertake the collection of pathological specimens

i. Provide travel medicine services

j. Request pathology services as necessary

k. Process and interpret pathology and other test results as required

l. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

m. Maintain accurate clinical records in conjunction with extant legislation

n. Ensure SNOMED CT codes are used effectively

o. Maintain chronic disease registers

p. Develop, implement and embed an effective call/recall system

q. Be responsible for the implementation of an effective immunisation programme

r. Chaperone patients where necessary

s. Prioritise health issues and intervene appropriately

t. Support the team in dealing with clinical emergencies

u. Recognise, assess and refer patients presenting with mental health needs

v. Implement vaccination programmes for adults and children

w. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

x. Contribute to practice targets (QOF, etc.), complying with local and regional guidance

y. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

z. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

aa. Support the clinical team with all safeguarding matters in accordance with local and national policies

bb. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

cc. Deliver opportunistic health promotion where appropriate

dd. Help the clinical staff with Home visits

ee. Able to refer appropriately to secondary care or organize emergency admissions

ff.Able to help with clinical letters which require auctioning/filing

  • * Registered Nurse with Nursing and Midwifery Council
  • * Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
  • * Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
  • * Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
  • * Meets the standards for registered ANP working at advanced level
