Job summary
We are recruiting for aHCA, to work 15-18
hours per week on a permanent contract. The role will include independently undertaking home
visits for the PCN Enhanced Care Team.
This role will require an Enhanced
DBS Clearance. Should you have this certificate as part of the DBS
subscription service, we would be happy to accept this providing the original
is shown. Otherwise, you will be expected to complete a new application.
Main duties of the job
The successful applicant will independently complete Long-term condition reviews and Frailty Assessments. They will also help the team with demand for phlebotomy & HCA services.

of approximately 53,000 patients with a diverse demographic.

Working
in the community with housebound patients under the direction of a qualified
nurse.
To
collect venous/capillary blood samples without harm to patient or self by
following the phlebotomy standard operating procedures.
To
obtain such blood samples to a standard fit for laboratory testing ensuring
collection, maintaining appropriate files & records as required.
To
ensure that all phlebotomy equipment and accommodation is properly maintained
and suitable for use.
To
competently complete Frailty assessment visits.
To
competently complete Long Term Condition reviews.

Knowledge and Skills

- Be able to prioritise their day and be familiar with all routine blood tests and understand their clinical importance
- Strong communication in English, both written and spoken.
- To deal with distressed patients in an empathic, caring and compassionate way.
- To write and maintain accurate clinical records.
- Strong IT skills
