Job summary
We are recruiting for a Senior Healthcare Assistant or Nursing Associate to join the Enhanced Care Team, which provides support to frail housebound patients and those in care homes.
Main duties of the job
This role includes undertaking home visits, frailty assessments and providing phlebotomy and other HCA services. The role requires the use of your own vehicle with business insurance for home visits.
This role will require anEnhanced DBS Clearance. Should you have this certificate as part of the DBS subscription service, we would be happy to accept this providing the original is shown. Otherwise, you will be expected to complete a new application.
As the role involves patient home visits, you are required to own vehicle for work and have Business Car Insurance.

Job Title-HCA or Nurse Associate for the Enhanced Care Team, Bournemouth East Primary Care Network
Contract Type-Permanent
Hours:up to 25 hours per week. Full or part time. Working pattern negotiable.
Main duties and responsibilities (this list is not exhaustive):
- Undertake Frailty Assessments using the Dorset Care Plan/PCSP
- Signposting to other services/social prescribers/voluntary sector as appropriate.
- Conduct long-term condition housebound reviews
- Provide phlebotomy service
- Perform basic patient observations, wheelchair assessments, pain assessments, ECGs and other health checks
- Maintain accurate clinical records via SystmOne
- To work within agreed policies and procedures to promote safe clinical practice
Training will be provided in these areas, depending on experience. We are also able to support NVQ qualification.

Essential
- NVQ level 2 or Level 3 in Health and Social Care OR NMC registered Nursing Associate
- Qualified in Phlebotomy
- GCSEs or similar, to include English Language and Maths
Experience

- Ability to deal with distressed patients in an empathic, caring and compassionate way.
- To have their own vehicle and hold a clean driving licence to meet the travel requirements of the role. You will require business insurance
- Ability to write and maintain accurate clinical records
- Have strong IT skills
- Good time management skills
- Be able to prioritise their day and be familiar with all routine blood tests and understand their clinical importance
- To be able to work independently within a team structure and possess excellent communication skills
- Demonstrate a flexible approach to work and to follow instructions and guidance given to them
