Job summary
We are looking for proactive, enthusiastic Clinical Pharmacist to
join the PCN, on a permanent full/part time contract.
You will need to work well in a team and have excellent
communication skills in written and spoken English.
You will be expected to demonstrate a level of leadership
and be involved in the development of the role.
You must be able to prioritise your work and perform
efficiently without constant supervision and have at least one years post
registration experience.
Applications from candidates with either community
pharmacy or secondary care experience are welcome.
Education and supervision are provided to all team
members by a senior pharmacist and a GP.
Enrolment in the 18-month CPPE Pathway is expected for
all pharmacists. However, pharmacists with relevant experience may be eligible
for exemptions to certain modules of the pathway.

Main duties of the job
Our Clinical Pharmacists enjoy a varied role that include daily pharmacist clinics with face to face and telephone appointments. The Pharmacy team provide support with long-term condition management and perform structured medication reviews. Once in post, pharmacists will also work closely with our pharmacy technicians on prescribing improvements such as audits, quality initiatives and the development of relevant policies.
The network Board is committed to the development of the
team and want to see it expand and develop their roles. Pharmacy are regarded
as an essential team by all the practices. To underline the Board's commitment
to the team, we are provided with ring-fenced GP supervision time every two
weeks.

to develop their role and will enrol on the Primary Care Pharmacist Education
Pathway (PCPEP).
Salary - Important
The salary on offer is Band 7, between £43,742 - £48,000
per annum (FTE) DOE for pharmacists who have not completed the PCPEP.
For completion of the PCPEP and prescribing
qualification, the salary on offer will be up to £50,952.00 per annum (FTE)
DOE.

line with SOPs. This will involve reviewing patients records, amending records
and informing patients if required.
- Support the achievement of the
PCNs prescribing Quality and Outcomes Framework (QoF) targets.
- Undertake Quality Improvement
initiatives and agreed medication audits, keeping records, and undertaking
further cycles as required.
- Assist in the appropriate
monitoring of prescribing budgets and to liaise with the Medicines Optimisation
team at NHS Dorset
- Implementation of local and
national guidelines and formulary recommendations
- Advise on medicines supply
issues as appropriate. This will involve liaison with local Community
Pharmacies and patients.
