Job summary

Andover Primary Care Network is looking for an Advanced Nurse Practitioner (ANP) who will work autonomously within their professional scope of the practice providing care for patients face to face, telephone consultations and home visits.

You will need to be an enthusiastic, caring and motivated professional with excellent clinical, organizational & communication skills, who has the ability to work autonomously, responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The advanced nurse practitioner will preferably have an independent prescribing qualification and will be responsible for several clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Main duties of the job

The post holder will work collaboratively with the general practice team across the 5 practices within the PCN and the extended primary care team to meet the needs of patients. They will support the delivery of policy and procedures, providing nurse leadership. They will look to develop new and innovative concepts, models, methods and practices in order to deliver new and improved primary care services to meet the needs of the PCN population.

About us

Primary Care Networks form a key building block of the NHS long-term plans. Bringing general practices together allow us to provide a wider range of services to patients, whilst also decreasing the workload on GPs. This improves the ability of practices to recruit and retain staff, manage finances and facilities more efficiently, and to more easily integrate with the wider health and care system.

Andover Primary Care Network is a collaborative project involving the following GP Surgeries: Adelaide Medical Centre, Andover Health Centre Medical Practice, Charlton Hill Surgery, Shepherds Spring Medical Centreand St Marys Surgery.

As we come out of the Covid Pandemic and demand pressures on primary care continue to grow, the time for reorganisation of the NHS and a political appetite for transformation of services is increasing. Primary Care in Andover suffers these pressures on top of longer standing issues of socio-economic deprivation and the resulting health problems, which have been exacerbated by the pandemic.

The PCN offers additional roles working across practices, providing alternatives for patients to seeing a GP. Using pharmacists and care-coordinators to proactively manage our elderly and complex patients. We expect the PCN to continue to develop and improve patient care in Andover, and to create opportunities for innovative ways of promoting wellbeing.

Overview of Key Responsibilities

The advanced nurse practitioner will be working across the Andover Primary Care Network doing a combination of practice work and at the Health Hub delivering the following key responsibilities:

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team

Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action

Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice

Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person

Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.

As an ANP youll be working at a masters level aware or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies.

Primary Responsibilities

As an ANP you will be an experienced clinician who will provide care to patients registered at the surgery. This includes telephone triage and F2F assessments.

Develop, implement and embed health promotion and well-being programmes.

Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.

Diagnose and manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients

Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / highly specialised care advice beyond the scope of normal nursing practice e.g. care treatment plans, making referrals as appropriate (including fast track referrals)

Support the team in dealing with clinical emergencies

Recognise, assess, and refer patients presenting with mental health needs

Contribute to local and national practice targets clinical remit e.g. QOF, prescribing incentive scheme and national benchmarking.

Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

Support the clinical team with all safeguarding matters for children and vulnerable adults, in accordance with local and national protocols

Understand PCN and local policies for substance abuse and addictive behaviour, referring patients appropriately

Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, nation and practice protocols and within the scope of qualified practice

Management of health screening programmes and ensure other team members are aware of health priorities and screening objectives and programmes.
