Job summary

To provide care to patients presenting with a minor illness, taking initial history, clinical assessment, diagnosis, treatment and evaluation of care. The clinician will prioritise and triage the needs of patients, accordingly, making necessary referrals for investigations in the appropriate manner. Patients with problems needing referral to secondary care would be discussed with a GP in the first instance. The ANP would work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedure.

The ANP may provide home visits when clinically necessary.

The ANP will deliver evidenced-based care to patients presenting with long term conditions and provide preventative healthcare.

Main duties of the job

Assess, diagnose, plan, implement and evaluate treatment / interventions and care for patients presenting with minor illness and/or long term conditions.

Clinically examine and assess patient need and plan clinical care accordingly.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition.

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness.

Work with patients in order to support compliance and adherence to prescription treatments.

Provide information and advice on prescribed or over-the-counter medication on medication regimes, side effects and interactions.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies to promote healthy lifestyles and apply principles of self-care.

Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings / reports.

Refer patients to other members of the primary and secondary health care teams as appropriate.

Ensure clinical practice is safe and effective and remains within the boundaries of competence and acknowleged limitations.

We are looking for an enthusiastic, committed ANP to join our dedicated practice team. We are a well-established practice based in the heart of Letchworth Garden City with a list size of approx 11,700 patients.

Letchworth town offers direct train links to both London and Cambridge, each taking only approximately 30 minutes to reach. Schooling in the town is very sought after and leisure facilities are outstanding.

We pride ourselves on delivering professional medical care in a friendly environment while striving to meet all the challenges of modern General Practice. We are members of the local Primary Care Network providing extended services as a hub to the local community.

At Birchwood (main surgery) there are a team of salaried GP's, Paramedic, Nurses, Health Care Assistants and Clinical Pharmacists who provide care and support to approx 11,700 patients.

Extended Access operates at Sollershott surgery (branch surgery), providing out of hours appointments Monday - Saturday for patients.
