Job summary

An exciting new position has been created at Church Stretton Medical Practice. We are looking for an enthusiastic and resilient person to join our clinical team as a Prescribing Pharmacist.

Applicants will need a pleasant, approachable manner, be a team player, have computer skills, good communication skills and the ability to remain calm whilst busy. Whilst previous experience of working in a Primary Care environment would be an advantage, it is not essential. The appropriate training will be provided for the right applicant.

The successful applicant will be employed by the SW Shropshire Primary Care Network, but the role will be based solely at Church Stretton Medical Practice.

Main duties of the job

To deliver pharmaceutical support to the clinical team at the medical practice supporting safe prescribing, medicines safety, long-term condition management, antimicrobial stewardship and structured medication reviews.

Interviews will take place on the afternoon of Tuesday 16 December 2025

Church Stretton medical practice is situation at the bottom of the Stretton hills. It is a rural practice with approximately 7,200 patients.

We are a 6 GP partner, 2 salaried GP, 6 nurse practice supporting by HCAs and an administration team.

We are a friendly, professional and busy practice.

Job responsibilities

Undertake structured medication reviews (SMRs) in line with the contractual requirements of the PCN DES for targeted cohorts of patients that would most benefit, including those most at risk of experiencing health inequalities.

Employ medicines optimisation strategies for reducing polypharmacy, minimising risk of prescribing harm, reducing over-prescribing and managing the risk of dependency on prescription drugs.

Be a prescriber, or in the process of completing the training to become a prescriber, working alongside the Practice clinical team.

Implement changes to medicines in line with MHRA alerts, product withdrawal or shortage and other local or national guidance.

Provide clinical leadership on medicines optimistion, including high-risk drugs, repeat prescribing and deprescribing

Support the management of patients with chronic diseases such as diabetes, asthma, hypertension and heart failure, in line with national and local guidelines. Including seeing patients in Chronic Disease clinics, starting and titrating medicines.

Proactively manage medicines-related risk, including contributing to medicines safety and incident investigations.

Experience

Essential

  • Significant post-qualification experience.
  • Experience of working with the elderly and patients with long term or chronic conditions.

Desirable

  • Previous experience of working in Primary Care.
  • Previous experience of the EMIS system
  • Experience of working face to face with patients.

Qualifications

Essential

  • Masters degree in pharmacy or equivalent
  • Mandatory registration with the General Pharmaceutical Council.
  • Independent prescriber or currently working towards completion of the qualification.
  • Postgraduate clinical qualification and/or significant relevant clinical experience to demonstrate similar level of knowledge

Knowledge and skills

  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
  • An appreciation of the nature of GPs and general practices.
  • An appreciation of the nature of primary care prescribing, concepts of national prescribing and strategies for improving prescribing.
  • Excellent interpersonal skills
  • Excellent written and verbal communication
  • Able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Able to obtain and analyse complex technical information.
  • Recognises priorities when problem solving and identifies deviations from the normal patterns and is able to refer to the relevant person when appropriate.
  • Wide range of IT skills including telephone consultations
