Job summary

We are looking for a caring, motivated individual to join our friendly GP practice in a dual role combining Health Care Assistant (HCA) duties with Care Navigator and Administrative support.

This is a varied and rewarding position supporting both the clinical team and our patients from taking bloods and conducting health checks to helping patients access the right care at the right time. You'll work across both clinical and reception/admin areas, making a real difference in patients healthcare journeys.

Main duties of the job

Clinical (HCA) Duties:

  • Perform phlebotomy (blood taking)

  • Record observations (e.g. blood pressure, pulse, temperature)

  • Carry out ECGs and urinalysis

  • Support chronic disease monitoring and NHS Health Checks

  • Assist with wound care and dressings

  • Support vaccination clinics (e.g. flu) if trained

  • Maintain clinical areas and infection control standards

  • Accurately document all care in patient records

Care Navigation & Admin Duties:

  • Greet and support patients, signposting to appropriate services

  • Book appointments and manage patient recalls

  • Handle phone, in-person, and online queries

  • Liaise with external services and secondary care providers

  • Process prescriptions and medication queries (as appropriate)

  • Input and update patient data on clinical systems (SystmOne)

  • Assist with scanning, document workflow, and coding

  • Support the delivery of QOF and other clinical targets

Raj Medical Centre, based on Laceby Road in Grimsby, is a well-established GP practice serving a diverse patient population in an area of high socio-economic need. Job Summary:

We are looking for a versatile and compassionate individual to join our General Practice team as a Health Care Assistant & Care Navigator/Admin. This hybrid role combines clinical support duties with care navigation and administrative responsibilities, ensuring patients receive seamless, high-quality care throughout their journey with the practice.

The ideal candidate will be a team player with strong interpersonal and organisational skills, capable of working in both patient-facing and back-office environments.

Key Responsibilities:

Clinical (HCA) Duties:

  • Undertake clinical tasks as directed by the nursing and GP team, including but not limited to:
    • Phlebotomy (blood taking)
    • Blood pressure monitoring
    • ECGs
    • Urinalysis
    • NHS Health Checks
    • Wound care/dressing changes
    • Spirometry (if trained)
    • Supporting chronic disease reviews (e.g., diabetes, asthma) under supervision
  • Ensure accurate documentation of care provided in patient records
  • Assist with vaccination clinics (e.g., flu) as required and if trained
  • Prepare and maintain clinical areas, ensuring infection control standards are met
  • Support clinicians during minor procedures

Care Navigation & Administration:

  • Act as a first point of contact for patients, signposting them to the most appropriate service or clinician using care navigation protocols
  • Manage patient queries both face-to-face and via telephone or online systems
  • Book appointments, manage recalls, and assist with chronic disease monitoring processes
  • Liaise with external agencies and secondary care providers on behalf of patients
  • Support the repeat prescription process and medication queries, where appropriate
  • Ensure accurate and timely data entry and coding onto clinical systems (e.g., EMIS/SystmOne)
  • Contribute to maintaining patient registers and call/recall systems (e.g., QOF, NHS Health Checks)
  • Assist with document management (e.g., scanning, workflow, and filing correspondence)
  • Participate in audits, data quality improvement projects, and service evaluations
