Job summary

An exciting position as an ANP has become available to join our dynamic team at Fernlea Surgery. We are looking for a talented, enthusiastic, hardworking Nurse to provide excellent care using clinical solutions and to help with our excellent Patient Care and Service.

This is a Full Time Position or Part Time position.

Main duties of the job

Job responsibilities include:

  • Provide care for patients presenting with acute, non-acute and chronic medical conditions, as well as general nursing duties in collaboration with the wider nursing and clinical teams
  • Cytology
  • Wound care / ulcer care/Doppler
  • Removal of Sutures
  • New Patient Medicals/Urinalysis
  • Travel Clinic
  • Asthma/Peak Flow Readings
  • Venepuncture
  • Hypertension Management
  • Routine/Childhood Immunisations
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician
  • Assisting GPs with minor surgery and coil fittings
  • Requesting pathology tests, for example urine culture, swabs
  • Interest in Diabetes, Coronary Heart Disease and Contraception.
  • Following agreed clinical protocols with referral to GPs as appropriate
  • See paediatric patients below age 5, within ANP scope of work

Other tasks include:

  • Infection Control.
  • Maintaining Clinical rooms, stocking as required
  • A duty to advise of potential problems or errors within a range of assigned tasks
  • Attend and participate in any Practice meetings when required.
  • On occasions there may be a requirement to undertake home visits.

Fernlea Surgery can trace its roots back to the early 1900s. Legend has it that on the death of its founder, the streets of Tottenham were closed due to the masses of people who followed Dr Cronins coffin to his final resting place.

Fernlea has a growing list size, presently 12,000. There are 4 partners, 6 salaried GPs, 3-4 GP registrars, 1 Physicians Associate, 2 nurses, 2 HCAs and non-clinical staff. We truly value our staff and encourage them to adopt our ethos of lifelong learning and further develop their skills.

Patient care and experience drives our enthusiasm. Patients can access our services using a range of modalities with Healthwatch stating we have better access compared to others in the area. Appointments are 15 minutes with a number of specialised clinics. Our services are tailored to accommodate our diverse population. CQC inspections have always been rated Good.

Job description

Job responsibilities

Work within the professional Code of Conduct and other directives as defined by the NMC.

Work autonomously and be accountable for own professional actions.

Be responsible for remaining up-to-date with CPD requirements to maintain professional registration with the NMC; including revalidation every three years.

Ensure own educational commitment is at least sufficient to maintain CPD requirements and to ensure own mandatory training certificates are current, in line with organisational policy.

Provide clinical leadership, support and training to colleagues and students.

Demonstrate a clear understanding of the accountability and legal implications of the ANP role.

Participate in a regular performance review with the Senior GP, using an agreed competency framework appropriate to the individuals scope of practice.

Clinical Responsibilities

Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to registered patients.

Assess patients with a range of acute, non-acute, and chronic medical conditions.

Take a history, conduct appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow-up.

Highlight any gaps in knowledge/training needs to the Senior GP and allow them the opportunity to offer support in order to fulfil the duties of this post.

Remain up-to-date with clinical guidelines to ensure that practice remains evidence based; particularly guidelines for the management of chronic diseases and national/local guidance/ICB formularies when prescribing.

Where appropriate make referrals to other healthcare professionals in primary and secondary care and arrange hospital admission for applicable cases.

Attend MDT meetings and maintain effective relationships with multi-professional colleagues; such as the care co-ordinators, community palliative care team, district nurses, specialist nurses, mental health and social care.

Undertake consultations in the most appropriate setting, using various consultation types; including, but not limited to, face to face, video, telephone, e-consultation.

Undertake medication reviews in line with local medicines management guidelines and with the support of the locality pharmacist.

Ensure clinical practice is safe and effective and remains within the boundaries of competence.

Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary

Work in line with organisational policies and procedural guidelines.

Attend and participate in practice meetings as required

Take part in clinical audit to continually review and improve the quality of care provision.

Prescribe treatment in line with local and national guidelines and ICB formulary.

You will be required to undertake all other duties that may be reasonably asked of you.
