Job summary

We are looking for a qualified diabetic nurse specialist or a qualified registered nurse with a minimum of 2 years post qualification and an interest in diabetes who is willing to be trained to cover and potentially lead this area of chronic disease. The ideal candidate with have other practice nurse duties within the team.

Main duties of the job

We are looking for a qualified registered nurse to join our nursing team covering Chronic disease management and treatment room duties with the view to further down the line taking over Diabetic care with the support of the doctors and our existing Diabetic nurse.

About us

We are a GP surgery working within 3 practice sites, Dibden Purlieu, Marchwood and the urgent care centre in Hythe Hospital. We are looking for an additional qualified nurse with an interest in diabetes to compliment our nursing team of 2 ANP's, 4 practice nurses, 1 GP assistant and 1 HCA.

Clinical Duties

A holistic approach to patient care

The ability to implement and embed health promotion and well-being programmes into the practice

The ability to implement and evaluate individual treatment plans for our chronic disease patients (asthma, COPD, Hypertension)

Develop an in-house sustainable diabetes management service

The ability to identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health.

The experience to provide, routine nursing care to primary care patients following clinical based evidence, NICE & NSF

Implement vaccination programmes for adults and children

Cervical Screening

Oversee the HCAs with NHS health checks & health promotion, smoking cessation, weight management

The ability to provide, treatment room duties covering, wound care, ulcers and dopplers, chronic disease, vaccinations & immunisations, contraception & HRT, travel advice, ECGs, venepuncture and more.

Chaperone patients where necessary

Recognise, assess, and refer patients presenting with mental health needs.

Administrative Duties

  • Develop practice protocols and guidelines in conjunction with GPs.

Request pathology services as necessary

Refer patients on to a GP or secondary/specialist care where appropriate

Maintain accurate clinical records in conjunction with safeguarding 7 extant legislation ensuring patient records are used correctly and effectively

Maintain chronic disease registers to support QOF and improve patient care, maintaining, practice income.

Liaise with external services and agencies to ensure patients are supported appropriately

Support patients in the use of prescribed and over the counter medicines

  • Management of chronic disease through an Accurx Florey where appropriate
  • To order and maintain our stocks of vaccines and injectables personally administered by the Partners or Practice Nurses.
  • The monitoring of practice fridge temperatures
