Job summary

We are seeking a motivated and compassionate Practice Nurse to join our friendly and forward-thinking team at The Wall House Surgery. This is an exciting opportunity to contribute to the provision of high-quality nursing care within a supportive and collaborative primary care environment.

The successful candidate will work as part of our multidisciplinary team, supporting the delivery of a wide range of nursing services to our registered patient population. You will provide evidence-based, patient-centred care in line with national guidance, local protocols, and practice policies, either independently or in partnership with our GPs and wider clinical team.

Main duties of the job

Clinical Care Delivery evidence-based care to patients with acute and chronic conditions, including diabetes, asthma, COPD, hypertension, and cardiovascular disease.Provide nursing treatments including wound care, immunisations (child and adult), cervical screening, ear irrigation, contraceptive advice, and phlebotomy.

Conduct long-term condition reviews, new patient health checks, ECGs, blood pressure monitoring, and spirometry (training can be provided).Provide pre-travel health advice and administer relevant travel vaccinations. Assist in LARC and minor surgery clinics as required.

Job description

Job responsibilities

  • Deliver evidence-based care to patients with acute and chronic conditions, including diabetes, asthma, COPD, hypertension, and cardiovascular disease.

  • Provide nursing treatments including wound care, immunisations , cervical screening, ear irrigation, contraceptive advice, and phlebotomy.

  • Conduct long-term condition reviews, new patient health checks, ECGs, blood pressure monitoring, and spirometry (training can be provided).

  • Support the delivery of NHS and local screening and vaccination programmes.Participate in anticoagulation monitoring and DOAC initiation and review.

  • Provide pre-travel health advice and administer relevant travel vaccinations.

  • Assist in LARC and minor surgery clinics as required.

Health Promotion & Safeguarding

  • Contribute to the planning and delivery of health promotion and disease prevention initiatives aligned to population health needs.

  • Ensure up to date awareness of and actively promote services and support for patients such as smoking cessation, exercise programs and psychological support.

  • Ensure accurate use and referral to other services such as community nursing and pharmacy services where appropriate.

  • Recognise safeguarding concerns and make appropriate referrals (e.g. children, vulnerable adults, domestic abuse).

Teamwork and Leadership

  • Work collaboratively with clinical and non-clinical colleagues across the surgery and wider primary care network (PCN).

  • Support the supervision and education of student nurses and allied health professionals contributing to a positive learning environment.

  • Provide input into clinical audits, research, and quality improvement initiatives.

Record Keeping & Communication

  • Maintain accurate and timely records using EMIS Web and use appropriate coding/templates as per organisational policy.

  • Communicate effectively with patients, carers, and professionals across the health and care system.

  • Relay test results, information and health advice clearly and sensitively to patients.

  • Communicate effectively with patient and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Administrative & Professional Duties

  • Attend practice meetings, contribute to service development, and complete administrative tasks linked to clinical care.

  • Participate in protected learning time, statutory and mandatory training.

  • Check and respond to NHS email and Microsoft Teams communications during shifts.

  • Adhere to professional standards, data protection, and confidentiality at all times.

  • Support daily tasks such as managing clinical room stock, chaperoning, and infection control procedures.
