Job summary
Adderley Green Medical Services Ltd is looking for an additional Health Care Assistant to join our team. The role is part time, but full time may be considered.
You must be experienced in working in a General Practice to perform this role.
You will be working under the direct supervision of the clinical team and strictly in accordance with specific practice guidelines and protocols. The Healthcare Assistant will assist the practice clinical team in the provision and delivery of practice programmes of patient care.
This role also involves visiting patients in their own homes to perform health checks and carry out joint visits with the nursing team in the delivery of flu & covid vaccination programmes.
Main duties of the job
Health Checks - including new patient, frail elderly, lifestyle, diabetic reviews, and NHS Health Checks
Phlebotomy, Vaccinations, BP Checks, 24 hour BP, weight management, performing ECGs, simple wound care, removal of sutures, urine dip tests
Home visits to housebound and frail elderly patients, including joint home visits with the nursing team for vaccination programmes.
Participation in QOF, QIF and other local and national enhanced services.
Helping to raise awareness of health and well-being and how it can be promoted
Processing and management of laboratory samples requested by GPs/Nurses
Clearing own consulting room and re-stocking all clinical rooms,
Preparing and maintaining environments and equipment before, during and after patient care interventions
Assisting with the collection and collation of data on needs related to health and well-being
Particpation in clinical governance meetings and audit.
Maintaining training and knowledge base.

Job responsibilities
The post
holder will offer general assistance and support to the Practice Nursing Team,
liaising closing with the Senior Practice Nurse and the Lead GP.
The HCA will
be involved primarily in undertaking the new patient health checks, over 75
health checks, urinalysis, ECG recording, spirometry, lifestyle advice &
weight management, administering authorised vaccination chaperoning patients,
maintaining hygiene standards, maintenance of equipment and the recording of
refrigerator temperature on a daily basis, ordering of stock and re-stocking GPs
and Nurses treatment rooms, maintaining the emergency drug checklist. The HCA
will also be responsible for documenting his/her patient consultations on the
EMIS clinical information system in General Practice.
He/she will
maintain patient confidentiality and will work under the direct supervision of
the Senior Practice Nurse & lead GP and within Practice Guidelines and
Protocols at all times.
Clinical
Duties
To assist in restocking clinical area with supplies and health promotion
materials as agreed with GPs/Nursing Team and to restock all rooms as
appropriate.
Monitor emergency drugs check list
To assist in general nursing duties, under the supervision of the Practice Nursing Team, i.e. Urine
testing, weight/height, blood pressure measurements, ECG recording, spirometry,
peak flow measurements smoking cessation advice and any other duties that may
be assigned as necessary.
To ensure
that drug/vaccine refrigerators are at the correct temperature on a daily basis
and maintain the Vaccine Cold Chain documented record according to Practice
Guidelines, in the absence of designated Nurse.
To act as a
chaperone as required to GPs and Practice Nurses.
To
participate in the induction programme of staff new to the practice
To receive
and document telephone messages as and when necessary and communicate to other
members of the clinical team as appropriate.
To maintain
health information notice board in line with national/local health promotion
campaigns.
To undertake
appropriate formal training as required by the Practice.
Administer
B12, Influenza, Shingles, RSV & Pneumococcal vaccines under PSD.
Carry out home visits to perform health checks on the housebound and frail elderly population.
Support joint home visits with the nursing team for the delivery of vaccination programmes.
General

To function
as an integral part of the Primary Health Care Team.
To attend
relevant clinical meetings as required.
To have
knowledge of and act at all times within the boundaries of Infection Control & Health & Safety
