Job summary
Meir Primary Care Network are looking to recruit an enthusiastic & highly motivated Advanced Clinical Practitioner to join our growing PCN team. You will be working with our 6 member practices to provide high quality care to our 37,500 patient population in the Meir, Longton & Blythe Bridge areas of Stoke On Trent.
You will be responsible for the provision of routine and acute medical
services, working as part of the PCNs multidisciplinary team, delivering care
within their scope of practice to the PCN patient population. You will be required to work in a patient
facing capacity, autonomously in the community. You will provide a range of
services such as assessment, diagnosis, treatment, telephone triage and
referring patients appropriately, as well as supporting the PCN management team
in the reviewing of clinical policy and procedure.
Main duties of the job
The post holder will:
Provide planned or unplanned care
to patients as required, in accordance with clinically based evidence, NICE
guidance, the Enhanced Health in Care Homes Framework and the Direct Enhanced
Services contract of the Network.
Triage, examine, diagnose offer
treatment, or liaise on patient care with the most appropriate member of the
clinical team as required.
Maintain accurate clinical records
in line with legislation, ensuring read codes are used correctly
Review the effectiveness of the
treatment provided, making changes where necessary to improve patient outcomes
Recognise, assess and refer
patients presenting with mental health needs where appropriate
Acting in accordance with local
and National safeguarding policies and procedures and having an awareness of
vulnerable patients in our community
Ensure that they adhere to the
relevant patient group directives and local clinical pathways at all times
Understand practice and local policies for
substance abuse and addictive behaviour, referring patients appropriately
Deliver
opportunistic health promotion where appropriate

We are a friendly, growing Primary Care Network Multi Disciplinary Team with a patient population of 37,500 working for 6 member practices covering Meir, Longton & Blythe Bridge
We have an excellent PCN team including 2 pharmacists, pharmacy technician, physiotherapist, paramedic, health & wellbeing coaches, mental health practitioner, mental health support worker, dietitian and care co-ordinator.

- Work autonomously and be accountable for his/ her professional actions.
- To undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to registered patients.
- Assess patients with a range of acute, non-acute, and chronic medical conditions. To take history, conduct appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans ensuring relevant follow-up.
- Where appropriate make referrals to other healthcare professionals in primary and secondary care.
- Arrange hospital admission for appropriate cases.
- Undertake telephone triage work and provide telephone advice calls as appropriate.
- Prescribe medications as appropriate.
- Maintain accurate record keeping in Emis Web.
- Undertake regular medication reviews and medicines management.

- Masters degree required for qualification post December 2020

- Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
- Qualified Independent Nurse Prescriber on the NMC register
- Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
- Meets the standards for registered ANP working at advanced level
