Job summary
The University Medical Centre in Bath is looking for an enthusiastic, confident and reliable full-time patient care coordinator to join our friendly GP Practice team. This is a permanent role and we are looking for the successful candidate to join us as soon as possible.
Having worked in General Practice or in a similar health care organisation is helpful but not essential as full training will be provided.
The role involves face to face interaction with the public and over the phone as well as communication via email and online systems.
Hours and shifts will be discussed and agreed at the interview stage and the successful candidate will need to be available to work a flexible shift pattern to support a combination of early shifts from 08:00-16:00 and later shifts from 10:00-18:00 across a weekly rota schedule.
Starting salary will be based on experience from a minimum of £10.50 an hour and which will be reviewed at six months.
Main duties of the job
The Patient Care Coordinator will be a first point of contact at the practice, working alongside the Medical Receptionist team to offer support and advice to help signpost patients and service users to the appropriate health care services. The role will require becoming familiar with practice and local community services and will have contact with a wider team to ensure effective and efficient utilisation of current patient pathways.
The role will support the reception and general administrative team to help in the booking of patient appointments and will offer support and advice to signpost patients in a courteous, effective, and efficient way.
To provide general assistance to the general practice team and project a positive and friendly image to patients and other visitors, both in person and via the telephone.
The post-holder will work with patients whilst based at the University Medical Centre.
Main duties will include booking of appointments, taking telephone enquiries and ensuring the smooth running of the clinical day to day surgeries whilst working on the front desk in the Practice.
There will also be a variety of administrative tasks to include the registration of new patients, managing incoming and outgoing mail and communications, processing repeat prescriptions and processing data entry and recording information in line with Practice guidance and policy.

The Practice has a large patient population comprising mainly of University students and staff and we also have a small number of patients from the local residential population.

Entering new patient registrations on to the IT clinical system (TPP / SystmOne)
Update the clinical system for patient changes of registration status as notified.
Request patient registration deductions from the clinical system as appropriate
Undertake any other additional administrative duties appropriate to the post as requested by the Partners or the Practice & Business Manager.
