Main duties of the job
We are expanding our team to meet the needs of our
population. You will be working
alongside a multidisciplinary team of experienced GPs, Nurses, Clinical
Pharmacists and First Contact Practitioners.
Our aim with your role is to deliver proactive care of patients
that are at high risk of admission, such as chronic illnesses/long term
conditions, decompensating frailty and End of Life care. We envisage your role
being varied, including online and telephone triage, face to face same day
consultations and home visits both urgent and routine, supported by an
experienced team of clinicians.
As an Advanced Paramedic Practitioner. You will be seeing
individuals with undifferentiated and undiagnosed problems. Based on patients assessment and a
comprehensive physical and functional examination, utilising expert knowledge
and clinical judgement, you will be able to identify the potential diagnosis
and manage accordingly. You will also be
able to assess, plan and implement programmes of care and prescribe appropriate
medication in liaison with colleagues, to support the individual.
Contract:We are offering a 2yr contract of employment
with the practice initially, with a possibility of this moving to employment
with the PCN
