Social Prescribing Link Worker

Job summary

We are seeking a Social Prescribing Link Worker to empower patients to take control of their health and wellbeing through personalised support. You will work closely with GP practices, local agencies, and community partners to connect patients with resources, statutory services, and local support networks.

This role involves:

  • Supporting people on sick leave to regain independence and return to employment.
  • Helping patients overcome health or social barriers to work and participation.
  • Supporting and developing neighbourhood-level projects that strengthen local community networks and resilience.

Main duties of the job

Social Prescribing

  • Receive referrals from GP practices local agencies and self referrals
  • Provide one to one personalised support focusing on what matters to me
  • Co produce simple care and support plans and link patients to community groups services and local resources
  • Build partnerships with voluntary community and social enterprise organisations to enhance capacity and sustainability
  • Promote social prescribing across the PCN and community providing guidance to staff on available services
  • Maintain accurate records track outcomes and adhere to safeguarding and confidentiality standards

Community and Employment Support

  • Support patients on sick leave to regain independence and return to work
  • Encourage engagement with local services training volunteering or other opportunities to increase employability
  • Identify barriers to engagement and help patients access practical and emotional support
  • Contribute to local neighbourhood projects working with community partners to strengthen networks and improve access to services

  • Organised, proactive, and able to manage a caseload independently.
  • Ability to collaborate with GP practices, local agencies, and community partners.
  • Commitment to equality, diversity, and social inclusion.
  • Ability to provide practical support that enhances employability, independence, and wellbeing.
  • Ability to conduct sensitive discussions with discretion and empathy.

Qualifications

  • Must be able to work in person, and live locally or be willing to relocate
  • NVQ Level 3 or equivalent or working towards.
  • Experience in community development, adult health/social care, or public health-paid or voluntary.
  • Experience supporting patients with complex social, health, or employment-related needs.

Desirable

  • Knowledge of local employment support services and pathways for people at risk of unemployment.
  • Training in motivational coaching, strength-based approaches, or behaviour change techniques.
  • Experience supervising volunteers or measuring service impact.
