Job summary
CAM Medical Primary Care Network has an exciting opportunity for a clinical pharmacist to join our team working across three GP practices in Cambridge City Centre. The clinical pharmacist will work across the CAM Medical Primary Care Network area in our pharmacy team. Work undertaken at the practices will be determined according to their existing and developing systems and will include some or all of the responsibilities noted below.
Medication Review and Optimisation
Care Home Medication Reviews
Long Term Condition Clinics
Medicines Reconciliation
Unplanned Admission Prevention
Medicines Information / Education
Prescribing Systems and Policies
Medicines safety
Liaison with community and hospital pharmacies
Main duties of the job
It is a requirement of the role to be registered with the General Pharmaceutical Council and have a minimum of 2 years of post-registration experience. Primary care experience would be an advantage, however, is not essential. Flexibility, enthusiasm and good clinical knowledge are essential. Successful candidates will be supported in terms of clinical learning and professional development through access to a GP clinical supervisor, other clinical pharmacists and enrolment onto a formal education pathway delivered through the Centre of Postgraduate Pharmacy Education (if not already completed).
