Job summary

Care coordinators review patient's needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.

Main duties of the job

Provide exceptional patient care services to patients in a fast-paced practice environment.

Develop and implement strategies to ensure patient satisfaction and improve overall patient experience.

Coordinate with healthcare professionals to ensure patient safety and compliance with regulations.

We are a friendly GP surgery looking for an enthusiastic and experienced care coordinator/receptionist who is looking to progress their career in General Practice. We have a very supportive, flexible team and pride ourselves on strong team working. Full or part time hours to be negotiated.
