Job summary
A patient care coordinator (PCC) may be required to deal with patients and, if appropriate,
their carers, before or after the patients consultation with a clinician or other healthcare
professional.
Working closely with the patient and their clinician or other healthcare professional in
particular the social prescribing link worker (SPLW) or social prescribing service provider
and health and wellbeing coach(es), the PCC co-ordinates patients healthcare and directs
them to the appropriate service to ensure that they get the most suitable care from
whatever health or social care provider is appropriate.
The PCCs role requires them to be able to work with, and understand the roles of, a
variety of different people working in the practice and across the PCN including doctors,
nurses, healthcare assistants, social prescribing link workers, physiotherapists, physician
associates, paramedics, health and wellbeing coaches, podiatrists, occupational
therapists and pharmacy technicians.
They may be given a caseload of identified patients and be required to ensure that their
changing needs are addressed by taking into account local priorities, health inequalities
and/or population health management risk stratification.
Main duties of the job
The primary responsibilities of a Personal Care Coordinator
(PCC) include utilizing population health intelligence to proactively identify
and work with patients for personalized care, supporting patients in using
decision aids for shared decision-making, creating personalized care plans,
assisting patients in managing their needs, supporting training and employment
opportunities, assessing activation levels and providing appropriate support,
facilitating access to self-management education and peer support, exploring
personal health budgets, coordinating care across services, managing caseloads
and referrals, raising awareness of shared decision-making, and maintaining a
clean and effective working area. Additionally, the PCC may be involved in
delivering quality initiatives, performing tasks aligned with the role, and
contributing to public health campaigns.
