Job summary

This is an excellent opportunity for someone to join our friendly team in a forward thinking modern healthcare setting.

The role will fit within our existing team of x2 ANPs and x 5 GPs, supported by the wider team of Pharmacist, Mental Health Practitioners, Paramedic, Practice Nurses, HCAs, Social Prescribers and Health and Well-Being Coaches.

There is the opportunity to tailor the job to best suit an individual's skills sets and ambitions, fitting in alongside the clinical team with their own varying skills and specialisms.

Exposure to working in a Primary Care setting would be ideal. General practice can be a fast pace way of working, but offers great variety and development opportunities.

The team support is second to none, and whilst we work hard, we are also a supportive and caring team who look out for one another and are continuously looking at ways to support staff and their well-being.

Main duties of the job

You will be responsible for clinically assessing and managing patients with a wide range of health conditions.

Typically, the ANP/ACP team will consult with patients for acute minor illness, routine pre-booked appointments and medication reviews. Some of our team specialise in gynae, or LTC management, and also Care Home ward rounds and support.

Admin time is allocated to support associated tasks, and breaks are factored into the rota.

Patient consultations are either in-person at the surgery, or remote by telephone.

Working pattern can be negotiated part time or full time, within the hours of 7.30am to 6.30pm, Monday to Friday.

Heartwood Medical Practice is a well-established and highly regarded GP Practice, supporting almost 11,500 patients from our modern premises in the centre of Swadlincote. CQC rated "Good" at our last review.

Heartwood belongs to the Swadlincote PCN group of 5 surgeries, and have access to shared additional ARRS funded roles to support us in practice.

We are an innovative practice, continuously seeking improvements in delivering high quality patient care.

We are also a Teaching Practice for medical students from the University of Nottingham and share the mentoring across our GP team.
