Job summary
This is an excellent opportunity for someone to join our friendly team in a forward thinking modern healthcare setting. As this is a new, additional role, to support growth of our practice, we have the opportunity to mould the job to best suit an individual's skills sets and ambitions, fitting alongside the clinical team with their own varying skills and specialisms.
The role will fit within our existing team of x3 ANPs and x 5 GPs, supported by the wider team of Pharmacists, Mental Health Practitioners, Paramedic, Practice Nurses, HCAs, Social Prescribers and Health and Well-Being Coaches.
Exposure to working in a Primary Care setting would be ideal. General practice can be a fast pace way of working, but offers great variety and development opportunities.
The team support is second to none, and whilst we work hard, we are also a supportive and caring team who look out for one another and are continuously looking at ways to support staff with well-being initiatives.

Main duties of the job
You will be responsible for clinically assessing and managing patients with a wide spectrum of clinical conditions.
Typically, the ANP team will consult with patients for acute illness, routine pre booked appointments, Long term conditions and medication reviews. Some of our team specialise in gynae, or LTC management, and Care Home support.
Admin time is allocated to support associated tasks, and breaks are factored into the rota.
Patient consultations are either in-person at the surgery, or remote by telephone or video.
Working pattern can be negotiated part time or full time, within the hours of 7.30am to 6.30pm, Monday to Friday.

Heartwood belongs to the Swadlincote PCN group of 5 surgeries, and have access to shared ARRS funded roles to support us in practice.
We are an innovative practice, continuously seeking improvements in delivering high quality patient care.
We are also a Teaching Practice for medical students from the University of Nottingham and share the mentoring across our GP team.
