Chronic Disease (Diabetes) Practice Nurse

Job summary

The post holder will be responsible for the delivery of General Practice Nursing to the whole Practice population (around 19,000 patients), as well as delivering specialist Chronic Disease Management to diabetic patients, providing and maintaining a high standard of nursing care for patients, as well as supporting the doctors and other members of the primary healthcare team.

The focus of this role is both the delivery of evidence-based practice for patients presenting with a long-term condition and the provision of preventative health care to the patients.

As an autonomous practitioner, the Chronic Disease Practice Nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making in the management of patients. They will work collaboratively within the Practice team to meet the needs of the patients, support the delivery of policy and procedures.

This role will specifically involve care of patients with diabetes and hypertension.

Main duties of the job

Assist in and perform routine tasks related to patient care as directed by senior staff and GPs

  • Cytology
  • Wound care / ulcer care / Doppler
  • Removal of sutures
  • Cervical Screening
  • New Patient Medicals / Urinalysis
  • Travel Vaccinations
  • ECGS
  • Chronic Disease Management including Diabetic and hypertension.
  • Venepuncture
  • Routine Immunisations / childhood immunisations.
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician.
  • Assisting GPs with Minor Surgery and coil fittings.
  • Requesting pathology requests, for urine culture and swabs.
  • Interest in contraception.
  • Following agreed clinical protocols with referral to senior nurses or GPs as appropriate.

Other tasks will include (but not limited to):

  • Maintaining and cleaning equipment.
  • Maintaining nurses tooms, stocking and rotating items as required
  • Chaperoning and assisting patients who are being examined by another clinician.
  • Maintain general tidiness and cleanliness of nurses and treatment rooms.
  • Ordering stock inc. ordering of vaccinations to maintain stock levels.
  • Participation of administrative systems in the practice
  • Maintaining accurate records at all times

Attend and participate in any practice meetings when required.

We are a busy, committed team working together in a recently extended and refurbished Practice in Launceston, close to the beautiful beaches of the north coast of Cornwall and surrounded by moorland countryside. The successful candidate will work within the support team to the GPs, already comprising of a Nurse Practitioner, Minor Illness Nurse and 6Paramedics, in addition to our Nurses and HCAs. We have 6 Partners and 8 Salaried GPs (including remote GPs) with very experienced Management, IT, Reception, and Administration support. Additional allied professionals who work within our team are our Clinical Psychologists, Pharmacist, Physiotherapists and Social Prescriber. We are also proud to be a training practice.
